স্ট্রোকের পর হাত ও কবজির দুর্বলতা খুবই পরিচিত একটি আপার লিম্ব সমস্যা। অনেক রোগী স্ট্রোকের পর কবজি উপরের দিকে তুলতে পারেন না, আঙুল সোজা করতে বা হাত খুলতে অসুবিধা হয় এবং দৈনন্দিন জিনিস ধরা বা ছেড়ে দেওয়াও কঠিন হয়ে যায়। কবজি অনেক সময় নিচের দিকে ঝুলে বা ফ্লেক্সড অবস্থায় থাকে, যা দেখতে রিস্ট ড্রপ-এর মতো মনে হতে পারে।

তবে কবজি ঝুলে থাকা বা দুর্বল হওয়ার কারণ সবার ক্ষেত্রে এক নয়। ক্লাসিক্যাল রিস্ট ড্রপ সাধারণত রেডিয়াল নার্ভের সমস্যা থেকে হয়। রেডিয়াল নার্ভ কবজি ও আঙুল এক্সটেনশন করার জন্য গুরুত্বপূর্ণ কয়েকটি মাংসপেশিকে নিয়ন্ত্রণ করে। অন্যদিকে স্ট্রোক মস্তিষ্ক এবং সেন্ট্রাল মোটর পাথওয়েকে প্রভাবিত করে। ফলে শোল্ডার, এলবো, ফোরআর্ম, রিস্ট, হাত ও আঙুলসহ পুরো আপার লিম্বে দুর্বলতা বা মুভমেন্ট কন্ট্রোলের সমস্যা দেখা দিতে পারে।

কিছু বিরল ক্ষেত্রে হঠাৎ করে শুধু রিস্ট ড্রপই স্ট্রোকের প্রথম লক্ষণ হিসেবে দেখা দিতে পারে এবং এটি রেডিয়াল নার্ভ পালসির মতো মনে হতে পারে। তাই দুর্বলতা কখন শুরু হয়েছে, অন্য কোনো নিউরোলজিক্যাল উপসর্গ আছে কি না এবং সমস্যাটি কোন ধরনের নার্ভ পাথওয়ের সঙ্গে সম্পর্কিত, তা নির্ণয় করা অত্যন্ত গুরুত্বপূর্ণ।

তাই কবজি ঝুলে গেলেই এটিকে শুধু স্থানীয় নার্ভের সমস্যা ধরে নেওয়া ঠিক নয়। হঠাৎ নতুন করে রিস্ট বা হাত দুর্বল হয়ে গেলে, বিশেষ করে স্ট্রোকের সম্ভাবনা থাকলে, দ্রুত মেডিকেল মূল্যায়ন প্রয়োজন।

রিস্ট ড্রপ কী?

রিস্ট ড্রপ বলতে এমন একটি অবস্থাকে বোঝায় যেখানে রোগী সক্রিয়ভাবে কবজি উপরের দিকে তুলতে বা রিস্ট এক্সটেনশন করতে পারেন না, অথবা খুব সীমিতভাবে করতে পারেন। হাত উপরে ধরে রাখার চেষ্টা করলে কবজি নিচের দিকে ঝুলে থাকে, কারণ রিস্ট এক্সটেনশনের সঙ্গে জড়িত মাংসপেশিগুলো স্বাভাবিকভাবে কাজ করতে পারে না।

মূল কারণের ওপর নির্ভর করে আঙুল বা থাম্ব সোজা করতে, হাত খুলতে, কোনো বস্তু ধরতে এবং দৈনন্দিন কাজে হাত ব্যবহার করতেও সমস্যা হতে পারে।

রিস্ট ড্রপকে প্রধানত দুই ধরনের মেকানিজমে ভাগ করে বোঝা যায়:

  1. সেন্ট্রাল নিউরোলজিক্যাল উইকনেস:
    সমস্যার উৎস মস্তিষ্ক বা সেন্ট্রাল নার্ভাস সিস্টেমে, যেমন স্ট্রোকের পর।
  2. পেরিফেরাল নার্ভ ডিসফাংশন:
    রেডিয়াল নার্ভ বা রিস্ট ও ফিঙ্গার এক্সটেনসর মাংসপেশিকে সাপ্লাই করা অন্য পেরিফেরাল নিউরোলজিক্যাল স্ট্রাকচারে সমস্যা।

এই দুই ধরনের সমস্যাকে আলাদা করে শনাক্ত করা জরুরি, কারণ এদের ইনভেস্টিগেশন, প্রগনোসিস এবং রিহ্যাবিলিটেশন পদ্ধতি এক নয়।

স্ট্রোকের পর রিস্ট ও হাতের দুর্বলতা

স্ট্রোক তখন ঘটে যখন মস্তিষ্কের কোনো অংশে রক্ত সরবরাহ বন্ধ হয়ে যায় অথবা মস্তিষ্কে বা মস্তিষ্কের আশপাশে রক্তক্ষরণ হয়। মস্তিষ্কের কোন অংশ আক্রান্ত হয়েছে তার ওপর নির্ভর করে শরীরের বিপরীত পাশের ভলান্টারি মুভমেন্ট নিয়ন্ত্রণকারী পাথওয়ে ক্ষতিগ্রস্ত হতে পারে।

আপার লিম্বের মোটর পাথওয়ে আক্রান্ত হলে রোগীর হালকা আঙুলের মুভমেন্টের সমস্যা থেকে শুরু করে পুরো হাতের গুরুতর দুর্বলতা বা প্যারালাইসিস পর্যন্ত হতে পারে।

স্ট্রোকের পর আপার লিম্বে সাধারণত যেসব সমস্যা দেখা যেতে পারে:

  1. আক্রান্ত হাতে দুর্বলতা বা প্যারালাইসিস:
    শোল্ডার, এলবো, রিস্ট, হাত ও আঙুল বিভিন্ন মাত্রায় আক্রান্ত হতে পারে।
  2. রিস্ট এক্সটেনশন কমে যাওয়া:
    রোগী গ্র্যাভিটির বিপরীতে কবজি উপরের দিকে তুলতে কষ্ট পেতে পারেন।
  3. ফিঙ্গার এক্সটেনশন কমে যাওয়া:
    আঙুল সোজা করা বা হাত খুলতে সমস্যা হতে পারে।
  4. সিলেক্টিভ মোটর কন্ট্রোল কমে যাওয়া:
    রোগী একটি নির্দিষ্ট জয়েন্ট বা মাংসপেশিকে আলাদাভাবে নিয়ন্ত্রণ করে নড়াতে অসুবিধা অনুভব করতে পারেন।
  5. হাত খুলতে অসুবিধা:
    ভলান্টারি এক্সটেনশন দুর্বল হলে হাত আংশিক বা সম্পূর্ণ বন্ধ অবস্থায় থাকতে পারে।
  6. গ্রিপ অ্যান্ড রিলিজ ফাংশন কমে যাওয়া:
    কোনো বস্তু ধরতে পারলেও সেটি ইচ্ছামতো ছেড়ে দিতে সমস্যা হতে পারে।
  7. মাসল টোনের পরিবর্তন:
    কিছু রোগীর ক্ষেত্রে শুরুতে মাসল টোন কম থাকতে পারে এবং পরবর্তীতে নিউরোলজিক্যাল রিকভারির সঙ্গে টোন বেড়ে যেতে বা অস্বাভাবিক হতে পারে।
  8. সেনসেশন বা প্রোপ্রিওসেপশনের পরিবর্তন:
    টাচ, প্রেসার, জয়েন্ট পজিশন বা মুভমেন্ট ঠিকভাবে বুঝতে অসুবিধা হতে পারে।
  9. ফাংশনাল হ্যান্ড লিমিটেশন:
    খাওয়া, পোশাক পরা, গ্রুমিং, লেখা, ফোন ধরা, গ্লাস বহন করা বা বোতাম লাগানোর মতো দৈনন্দিন কাজে অসুবিধা হতে পারে।

তাই পোস্ট-স্ট্রোক হ্যান্ড ডিসফাংশন ক্লাসিক্যাল রেডিয়াল নার্ভ রিস্ট ড্রপের তুলনায় অনেক বিস্তৃত। স্ট্রোকে শুধু রেডিয়াল নার্ভের নির্দিষ্ট প্যাটার্ন নয়, বরং পুরো আপার লিম্বে উইকনেস ও মোটর কন্ট্রোলের সমস্যা দেখা দিতে পারে।

রিস্ট ড্রপ কি স্ট্রোকের প্রথম লক্ষণ হতে পারে?

যদিও এটি খুব সাধারণ নয়, কিছু ক্ষেত্রে আইসোলেটেড রিস্ট ড্রপ স্ট্রোকের কারণে হতে পারে।

মস্তিষ্কের হাতের মুভমেন্ট নিয়ন্ত্রণকারী মোটর এরিয়ার ছোট কোনো কর্টিক্যাল স্ট্রোকের কারণে হঠাৎ রিস্ট ও ফিঙ্গার এক্সটেনশন দুর্বল হয়ে যেতে পারে। যেহেতু দুর্বলতা কখনো কখনো শুধু হাতেই সীমাবদ্ধ থাকে, তাই প্রথমে এটি রেডিয়াল নার্ভ পালসি বলে মনে হতে পারে।

গুরুত্বপূর্ণ কিছু ক্লিনিক্যাল ক্লু:

  1. নার্ভ কমপ্রেশন বা ট্রমার স্পষ্ট ইতিহাস ছাড়াই হঠাৎ দুর্বলতা শুরু হওয়া।
  2. হঠাৎ রিস্ট বা ফিঙ্গার এক্সটেনশন বন্ধ বা উল্লেখযোগ্যভাবে কমে যাওয়া।
  3. হঠাৎ হাতের কন্ট্রোল কমে যাওয়া।
  4. একই সঙ্গে হাতের অন্য অংশ বা শরীরের অন্য অংশ দুর্বল হওয়া।
  5. অন্যান্য নিউরোলজিক্যাল উপসর্গ থাকা, যদিও সব ক্ষেত্রে এগুলো স্পষ্ট নাও হতে পারে।

স্ট্রোকের চিকিৎসা সময়নির্ভর। তাই কোনো স্পষ্ট পেরিফেরাল নার্ভ ইনজুরি ছাড়াই হঠাৎ রিস্ট ড্রপ হলে সেটিকে সাধারণ নার্ভের সমস্যা ধরে নিয়ে চিকিৎসা শুরু করা উচিত নয়।

জরুরি সতর্কতা

হঠাৎ রিস্ট ড্রপ বা হাতের দুর্বলতার সঙ্গে নিচের যেকোনো উপসর্গ থাকলে অবিলম্বে জরুরি মেডিকেল সেবা নিন:

  1. মুখের এক পাশ দুর্বল বা বেঁকে যাওয়া।
  2. কথা বলতে বা অন্যের কথা বুঝতে অসুবিধা।
  3. হাত বা পায়ে হঠাৎ দুর্বলতা।

  8. কনফিউশন, চেতনার পরিবর্তন বা অন্য কোনো নতুন নিউরোলজিক্যাল উপসর্গ।

অ্যাকিউট স্ট্রোক সন্দেহ হলে ফিজিওথেরাপি মূল্যায়নের জন্য অপেক্ষা না করে আগে জরুরি মেডিকেল মূল্যায়ন করতে হবে।

স্ট্রোকের পর রিস্ট ও ফিঙ্গার এক্সটেনশন কেন দুর্বল হয়?

ভলান্টারি হ্যান্ড মুভমেন্টের জন্য মস্তিষ্ক থেকে মাংসপেশি পর্যন্ত একটি ধারাবাহিক নিউরোলজিক্যাল যোগাযোগ প্রয়োজন।

সহজভাবে এই পাথওয়েটি হলো:

মস্তিষ্কের মোটর কর্টেক্স → কর্টিকোস্পাইনাল পাথওয়ে → স্পাইনাল কর্ড → পেরিফেরাল নার্ভ → মাংসপেশি

মস্তিষ্কের মোটর এরিয়া ভলান্টারি মুভমেন্টের কমান্ড তৈরি করে। এই সিগন্যাল বিভিন্ন ডিসেন্ডিং পাথওয়ের মাধ্যমে স্পাইনাল কর্ডে যায় এবং সেখান থেকে পেরিফেরাল নার্ভ হয়ে মাংসপেশিতে পৌঁছায়।

স্ট্রোকের কারণে মস্তিষ্ক বা মোটর পাথওয়ে ক্ষতিগ্রস্ত হলে এই সিগন্যাল বাধাগ্রস্ত হতে পারে। রিস্ট ও ফিঙ্গার এক্সটেনসর মাংসপেশি এবং রেডিয়াল নার্ভ নিজে অ্যানাটমিক্যালি অক্ষত থাকতে পারে, কিন্তু মস্তিষ্ক সেই মাংসপেশিকে স্বাভাবিকভাবে নিয়ন্ত্রণ করতে পারে না।

এটি পেরিফেরাল রেডিয়াল নার্ভ রিস্ট ড্রপ থেকে ভিন্ন।

রেডিয়াল নার্ভ লেশনের ক্ষেত্রে:

রেডিয়াল নার্ভ ইনজুরি বা কমপ্রেশন → পেরিফেরাল নার্ভ ট্রান্সমিশন ব্যাহত → রিস্ট ও ফিঙ্গার এক্সটেনসর মাংসপেশি দুর্বল

অর্থাৎ দুজন রোগীরই কবজি তুলতে সমস্যা হতে পারে, কিন্তু সমস্যার মূল নিউরোলজিক্যাল মেকানিজম সম্পূর্ণ আলাদা হতে পারে।

রিস্ট ড্রপের অন্যান্য কারণ

এই ব্লগে স্ট্রোকের পর রিস্ট ও হাতের দুর্বলতার ওপর বেশি গুরুত্ব দেওয়া হলেও ক্লাসিক্যাল রিস্ট ড্রপের সঙ্গে রেডিয়াল নার্ভ ডিসফাংশনের সম্পর্ক বেশি।

অন্যান্য গুরুত্বপূর্ণ কারণ হলো:

  1. রেডিয়াল নার্ভ কমপ্রেশন:

    রেডিয়াল নার্ভে দীর্ঘ সময় চাপ পড়লে নার্ভ কন্ডাকশন সাময়িক বা দীর্ঘমেয়াদে ব্যাহত হতে পারে। ফলে রিস্ট ও ফিঙ্গার এক্সটেনশনের জন্য দায়ী মাংসপেশি দুর্বল হয়ে যেতে পারে।

  2. রেডিয়াল নার্ভ ইনজুরি:

    আপার আর্ম, এলবো, ফোরআর্ম বা আশপাশের স্ট্রাকচারে ট্রমা হলে রেডিয়াল নার্ভ ক্ষতিগ্রস্ত হয়ে রিস্ট ড্রপ হতে পারে।

  3. হিউমেরাল শ্যাফট ফ্র্যাকচার:

    রেডিয়াল নার্ভ হিউমেরাসের খুব কাছ দিয়ে যায়। তাই কিছু হিউমেরাল শ্যাফট ফ্র্যাকচারে এই নার্ভ ইনজুরি বা ইরিটেশন হতে পারে।

  4. দীর্ঘ সময় বাহ্যিক চাপ:

    ঘুমের সময়, অচেতন অবস্থায়, ইনটক্সিকেশন বা দীর্ঘ সময় একই পজিশনে থাকার কারণে আপার আর্মে চাপ পড়ে রেডিয়াল নার্ভ কমপ্রেশন হতে পারে। একে অনেক সময় স্যাটারডে নাইট পালসি বলা হয়।

  5. ক্রাচ-সম্পর্কিত নার্ভ কমপ্রেশন:

    অ্যাক্সিলারি ক্রাচ ভুলভাবে ব্যবহার করলে আপার আর্ম বা অ্যাক্সিলারি অঞ্চলে অতিরিক্ত চাপ পড়ে পেরিফেরাল নার্ভ আক্রান্ত হতে পারে।

  6. রেডিয়াল নার্ভ এন্ট্রাপমেন্ট বা পোস্টেরিয়র ইন্টারঅসিয়াস নার্ভ ডিসফাংশন:

  8. অন্যান্য নিউরোলজিক্যাল বা সিস্টেমিক কন্ডিশন:

    কম ক্ষেত্রে অন্য কোনো নিউরোলজিক্যাল বা সিস্টেমিক সমস্যার কারণে রিস্ট ও হাতের দুর্বলতা দেখা দিতে পারে। অস্বাভাবিক প্যাটার্ন থাকলে বিস্তারিত ক্লিনিক্যাল মূল্যায়ন প্রয়োজন।

এই পার্থক্যগুলো দেখায় যে শুধু কবজির পজিশন দেখে চিকিৎসা দেওয়ার পরিবর্তে সমস্যার আসল কারণ শনাক্ত করা বেশি গুরুত্বপূর্ণ।

রিস্ট ড্রপ ও পোস্ট-স্ট্রোক হ্যান্ড উইকনেসের উপসর্গ

উপসর্গ রোগের কারণ এবং নিউরোলজিক্যাল ইনভলভমেন্টের মাত্রা অনুযায়ী ভিন্ন হতে পারে।

সাধারণ উপসর্গগুলো হলো:

  1. কবজি উপরের দিকে তুলতে না পারা বা কষ্ট হওয়া।
  2. আঙুল সোজা করতে অসুবিধা।
  3. হাত খুলতে সমস্যা।

  8. হাতের কো-অর্ডিনেশন ও ডেক্সটারিটি কমে যাওয়া।
  9. স্ট্রোকের পর হাত বা পুরো আপার লিম্ব দুর্বল হওয়া।
  10. অবশভাব, ঝিনঝিনি বা সেনসেশনের পরিবর্তন।
  11. হাতের পজিশন বা মুভমেন্ট বুঝতে অসুবিধা।
  12. কিছু পোস্ট-স্ট্রোক রোগীর ক্ষেত্রে মাসল স্টিফনেস বা টোন বেড়ে যাওয়া।

এখানে ফাংশনাল প্রভাব বিশেষভাবে গুরুত্বপূর্ণ। রিস্ট ও আঙুলের অল্প পরিমাণ কন্ট্রোল কমে গেলেও দৈনন্দিন স্বাধীনতা উল্লেখযোগ্যভাবে কমে যেতে পারে।

রিস্ট ড্রপের কারণ কীভাবে নির্ণয় করা হয়?

সঠিক ডায়াগনোসিস অত্যন্ত গুরুত্বপূর্ণ, কারণ সেন্ট্রাল নিউরোলজিক্যাল রিস্ট ড্রপ এবং পেরিফেরাল রেডিয়াল নার্ভ রিস্ট ড্রপের ম্যানেজমেন্ট এক নয়।

ক্লিনিক্যাল অ্যাসেসমেন্ট

মূল্যায়নের মধ্যে থাকতে পারে:

  1. উপসর্গ কখন ও কীভাবে শুরু হয়েছে তার বিস্তারিত ইতিহাস।

  6. সেনসরি পরীক্ষা।
  7. রিফ্লেক্স টেস্টিং।
  8. মাসল টোন মূল্যায়ন।
  9. আপার লিম্ব মোটর কন্ট্রোল ও কো-অর্ডিনেশন।
  10. ফাংশনাল হ্যান্ড অ্যাসেসমেন্ট।
  11. গ্রাসপিং, রিলিজিং, রিচিং এবং অবজেক্ট ম্যানিপুলেশন পর্যবেক্ষণ।

ইনভেস্টিগেশন

ক্লিনিক্যাল ফাইন্ডিং অনুযায়ী নিচের পরীক্ষা প্রয়োজন হতে পারে:

  1. ব্রেইন CT বা MRI:
    অ্যাকিউট স্ট্রোক বা অন্য সেন্ট্রাল নিউরোলজিক্যাল কারণ সন্দেহ হলে।
  2. নার্ভ কন্ডাকশন স্টাডি (NCS):
    পেরিফেরাল নার্ভ কন্ডাকশন মূল্যায়নে সহায়ক।
  3. ইলেক্ট্রোমায়োগ্রাফি (EMG):
    নার্ভ ও মাংসপেশির ফাংশন সম্পর্কে তথ্য দিতে পারে।
  4. মাসকুলোস্কেলেটাল ইমেজিং:
    ফ্র্যাকচার, ট্রমা বা স্ট্রাকচারাল ইনজুরি সন্দেহ হলে এক্স-রে, আল্ট্রাসাউন্ড বা অন্যান্য ইমেজিং করা হতে পারে।
  5. অতিরিক্ত নিউরোলজিক্যাল ইনভেস্টিগেশন:
    রোগীর উপসর্গ অনুযায়ী অন্য পরীক্ষা প্রয়োজন হতে পারে।

    বিশেষ করে শোল্ডারে দুর্বলতা বা সেনসেশন কম থাকলে আক্রান্ত হাত সাবধানে হ্যান্ডল করা প্রয়োজন।

  2. রেঞ্জ অব মোশন বজায় রাখা:

    অ্যাক্টিভ মুভমেন্ট সীমিত থাকলে প্রয়োজন অনুযায়ী প্যাসিভ বা অ্যাক্টিভ-অ্যাসিস্টেড মুভমেন্টের মাধ্যমে জয়েন্টের উপলব্ধ মোবিলিটি বজায় রাখা যায়।

    উপযুক্ত রোগীর ক্ষেত্রে অ্যাক্টিভ রিস্ট এক্সটেনশন, ফিঙ্গার এক্সটেনশন এবং হাত খোলার মুভমেন্ট ফ্যাসিলিটেট করার ওপর কাজ করা যেতে পারে।

    রোগী কতটুকু ভলান্টারি মুভমেন্ট করতে পারেন, মাসল টোন কেমন এবং অ্যাসোসিয়েটেড মুভমেন্ট আছে কি না, তার ওপর ট্রিটমেন্ট অ্যাপ্রোচ নির্ভর করে।

  5. স্ট্রেংথেনিং ও প্রগ্রেসিভ এক্সারসাইজ:

    রোগীর পর্যাপ্ত ভলান্টারি কন্ট্রোল থাকলে ধীরে ধীরে স্ট্রেংথেনিং এক্সারসাইজ শুরু ও প্রগ্রেস করা যেতে পারে।

    শুধু আলাদা মাংসপেশি নয়, ফাংশনাল কাজে প্রয়োজনীয় কো-অর্ডিনেটেড আপার লিম্ব মুভমেন্টের ওপরও গুরুত্ব দেওয়া উচিত।

  6. ফাংশনাল হ্যান্ড ট্রেনিং:

    ফাংশনাল প্র্যাকটিসে থাকতে পারে:

    • অবজেক্টের দিকে রিচিং
    • গ্রাসপিং
    • রিলিজিং
    • ছোট বস্তু তোলা
    • বস্তু ধরে রাখা ও বহন করা
    • অবজেক্ট ম্যানিপুলেশন
    • ইউটেনসিল ব্যবহার
    • পোশাক পরার সঙ্গে সম্পর্কিত কাজ

    রোগীর বাস্তব জীবনের ফাংশনাল গোলের সঙ্গে যতটা সম্ভব মিল রেখে এক্সারসাইজ নির্বাচন করা উচিত।

  7. বাই-ল্যাটারাল আপার লিম্ব ট্রেনিং:

    নির্বাচিত রোগীর ক্ষেত্রে দুই হাত একসঙ্গে ব্যবহার করে উপযুক্ত কাজ প্র্যাকটিস করা আক্রান্ত হাতকে ফাংশনালভাবে কাজে লাগাতে এবং রিপিটেটিভ প্র্যাকটিসে সহায়তা করতে পারে।

  8. সেনসরি ও প্রোপ্রিওসেপটিভ ট্রেনিং:

    স্ট্রোকের কারণে সেনসেশন বা প্রোপ্রিওসেপশন আক্রান্ত হলে রোগীর প্রয়োজন অনুযায়ী সেনসরি ইনপুট ও মুভমেন্ট অ্যাওয়ারনেস-ভিত্তিক কার্যক্রম অন্তর্ভুক্ত করা যেতে পারে।

  9. ইলেক্ট্রিক্যাল স্টিমুলেশন / NMES বা FES:

    উপযুক্ত রোগীর ক্ষেত্রে ক্লিনিক্যাল ইন্ডিকেশন থাকলে দুর্বল মাংসপেশি অ্যাক্টিভেট করতে নিউরোমাসকুলার বা ফাংশনাল ইলেক্ট্রিক্যাল স্টিমুলেশন ব্যবহার করা যেতে পারে।

    ইলেক্ট্রিক্যাল স্টিমুলেশন অ্যাক্টিভ রিহ্যাবিলিটেশন ও টাস্ক-স্পেসিফিক ট্রেনিংয়ের বিকল্প নয়, বরং প্রয়োজন অনুযায়ী সহায়ক অংশ হিসেবে ব্যবহার করা উচিত।

  10. টাস্ক-স্পেসিফিক ফাংশনাল রিহ্যাবিলিটেশন:

হোম এক্সারসাইজ রিস্ট ড্রপের মূল কারণ অনুযায়ী নির্বাচন করা উচিত। পোস্ট-স্ট্রোক রোগী এবং রেডিয়াল নার্ভ পালসির রোগীর জন্য এক্সারসাইজ ও প্রগ্রেশন সম্পূর্ণ ভিন্ন হতে পারে।

স্ট্রোকের পর হোম এক্সারসাইজ

নিচের কার্যক্রমগুলো নির্বাচিত রোগীর ক্ষেত্রে ফিজিওথেরাপিস্টের নির্দেশনায় ব্যবহার করা যেতে পারে:

  1. সাপোর্টেড রিস্ট এক্সটেনশন:
    ফোরআর্ম টেবিলের ওপর আরাম করে রেখে প্রয়োজন অনুযায়ী সহায়তা নিয়ে কবজি আস্তে উপরের দিকে তোলার চেষ্টা করুন। উপকারিতা: রিস্ট এক্সটেনসর মাংসপেশির কন্ট্রোলড অ্যাক্টিভেশন প্র্যাকটিস করতে সহায়তা করে।
  2. অ্যাক্টিভ-অ্যাসিস্টেড রিস্ট মুভমেন্ট:
    ভালো হাত দিয়ে আক্রান্ত রিস্টকে আরামদায়ক রেঞ্জে মুভ করতে সাহায্য করুন এবং একই সঙ্গে আক্রান্ত হাত দিয়ে সক্রিয়ভাবে মুভমেন্টে অংশ নেওয়ার চেষ্টা করুন। উপকারিতা: অ্যাক্টিভ কন্ট্রোল কম থাকলে মুভমেন্ট প্র্যাকটিসে সহায়তা করে।
  3. ফিঙ্গার ওপেনিং প্র্যাকটিস:
    উপলব্ধ মুভমেন্ট অনুযায়ী আঙুলগুলো আস্তে খুলতে চেষ্টা করুন, জোর করবেন না। উপকারিতা: ফিঙ্গার এক্সটেনশন ও হ্যান্ড ওপেনিং কন্ট্রোল প্র্যাকটিসে সহায়তা করে।
  4. রিচিং ও রিলিজ প্র্যাকটিস:
    হালকা কোনো বস্তুর দিকে হাত বাড়িয়ে সেটি ধরার চেষ্টা করুন এবং নিরাপদভাবে ছেড়ে দেওয়ার প্র্যাকটিস করুন। উপকারিতা: রিস্ট ও হাতের মুভমেন্টকে একটি অর্থপূর্ণ ফাংশনাল টাস্কের সঙ্গে যুক্ত করে।
  5. বাই-ল্যাটারাল হ্যান্ড অ্যাক্টিভিটি:

স্ট্রোকের পর এক্সারসাইজের রেপিটেশন বা ডোজ সবার জন্য এক নয়। মোটর কন্ট্রোল, মাসল টোন, ফ্যাটিগ, সেনসেশন, ব্যথা, কগনিশন ও ফাংশনাল অ্যাবিলিটির ওপর ভিত্তি করে এক্সারসাইজ প্রোগ্রাম নির্ধারণ করা উচিত।

পেরিফেরাল রেডিয়াল নার্ভ রিস্ট ড্রপের হোম এক্সারসাইজ

  1. প্যাসিভ রিস্ট এক্সটেনশন:
    ভালো হাত দিয়ে আক্রান্ত রিস্টকে সাপোর্ট করে আরামদায়ক সীমার মধ্যে আস্তে উপরের দিকে তুলুন। উপকারিতা: অ্যাক্টিভ এক্সটেনশন কম থাকলে রিস্ট মোবিলিটি বজায় রাখতে সহায়তা করে।

দুর্বলতা বেড়ে গেলে অথবা উল্লেখযোগ্য ব্যথা, অবশভাব, ঝিনঝিনি, ফোলা বা নতুন কোনো উপসর্গ দেখা দিলে এক্সারসাইজ বন্ধ করে পুনরায় মূল্যায়ন করা উচিত।

রিস্ট ড্রপ ভালো হতে কত সময় লাগে?

রিস্ট ড্রপের জন্য নির্দিষ্ট একটি রিকভারি টাইমলাইন নেই। কারণ রিকভারি সম্পূর্ণভাবে মূল নিউরোলজিক্যাল সমস্যার ওপর নির্ভর করে।

রিকভারিকে প্রভাবিত করতে পারে:

নিউরোলজিক্যাল রিকভারি দীর্ঘ সময় ধরে চলতে পারে। তাই নির্দিষ্ট সময়ের মধ্যে সেরে ওঠার প্রতিশ্রুতির পরিবর্তে রোগীর ব্যক্তিগত অগ্রগতির ভিত্তিতে রিহ্যাবিলিটেশন গোল নির্ধারণ করা উচিত।

রেডিয়াল নার্ভ ইনজুরি

রিকভারি নির্ভর করে:

  1. নার্ভটি কমপ্রেসড, স্ট্রেচড, আংশিক ইনজার্ড নাকি গুরুতরভাবে ক্ষতিগ্রস্ত হয়েছে।
  2. নার্ভ লেশনের লেভেল।
  3. অ্যাক্সোনাল ইনভলভমেন্টের মাত্রা।
  4. কমপ্রেশন কতক্ষণ ছিল।
  5. সঙ্গে ফ্র্যাকচার বা অন্য ট্রমা আছে কি না।

সার্জারি (যদি প্রয়োজন হয়)

কখন জরুরি মেডিকেল সেবা নিতে হবে?
