PCL ইনজুরি কী?

অতিরিক্ত ফোর্সের কারণে পোস্টেরিয়র ক্রুসিয়েট লিগামেন্ট স্ট্রেচ বা ছিঁড়ে গেলে তাকে PCL ইনজুরি বলা হয়।

PCL ইনজুরির একটি পরিচিত মেকানিজম হলো হাঁটু ভাঁজ করা অবস্থায় টিবিয়ার উপরের অংশে শক্তিশালী পেছনমুখী ফোর্স লাগা। উদাহরণ হিসেবে, সড়ক দুর্ঘটনায় হাঁটু ফ্লেক্সড থাকা অবস্থায় শিনের সামনের অংশ ড্যাশবোর্ডে আঘাত করলে টিবিয়া পেছনের দিকে ধাক্কা খেতে পারে।

ভাঁজ করা হাঁটুর ওপর সরাসরি পড়ে যাওয়া, হাঁটু অতিরিক্ত হাইপারফ্লেক্সন বা হাইপারএক্সটেনশনে যাওয়া অথবা খেলাধুলার সময় বড় ধরনের সংঘর্ষের কারণেও PCL ইনজুরি হতে পারে।

শুধু PCL-এ সীমাবদ্ধ ইনজুরি

এই ধরনের ইনজুরিতে মূলত PCL-ই প্রধানভাবে ক্ষতিগ্রস্ত হয় এবং হাঁটুর অন্যান্য গুরুত্বপূর্ণ লিগামেন্ট তুলনামূলকভাবে অক্ষত থাকে।

একাধিক স্ট্রাকচার যুক্ত PCL ইনজুরি

বেশি শক্তিশালী ট্রমায় PCL-এর সঙ্গে আরও কিছু স্ট্রাকচার একই সময়ে ইনজুরড হতে পারে, যেমন:

  • ACL
  • MCL
  • LCL
  • পোস্টেরোল্যাটারাল কর্নার বা PLC

PCL ইনজুরি অনেক সময় মাল্টি-লিগামেন্ট নি ইনজুরির অংশ হিসেবে দেখা যায়। তাই উল্লেখযোগ্য ট্রমার ক্ষেত্রে শুধু PCL নয়, পুরো হাঁটু জয়েন্টের স্ট্যাবিলাইজিং স্ট্রাকচারগুলো মূল্যায়ন করা গুরুত্বপূর্ণ।

PCL ইনজুরির ধরন ও গ্রেড

ক্লিনিক্যাল পরীক্ষায় টিবিয়া পেছনের দিকে কতটা অতিরিক্ত সরে যাচ্ছে বা পোস্টেরিয়র ল্যাক্সিটি কতটা রয়েছে, তার ওপর ভিত্তি করে PCL ইনজুরি সাধারণত বিভিন্ন গ্রেডে ভাগ করা হয়।

গ্রেড ১ PCL স্প্রেইন

গ্রেড ১ ইনজুরি তুলনামূলক হালকা।

এক্ষেত্রে থাকতে পারে:

  1. লিগামেন্ট ফাইবারে সীমিত ক্ষতি।
  2. হালকা পোস্টেরিয়র ল্যাক্সিটি।
  3. ব্যথা বা ফোলা।
  4. তুলনামূলক কম ফাংশনাল সমস্যা।

আক্রান্ত হাঁটুতে সুস্থ হাঁটুর তুলনায় প্রায় ০–৫ মিমি বেশি পোস্টেরিয়র ট্রান্সলেশন দেখা যেতে পারে।

গ্রেড ২ PCL টিয়ার

গ্রেড ২ ইনজুরিতে লিগামেন্টের ক্ষতি তুলনামূলক বেশি হয়।

এক্ষেত্রে থাকতে পারে:

  1. পোস্টেরিয়র টিবিয়াল ট্রান্সলেশন বৃদ্ধি।
  2. ব্যথা ও ফোলা।
  3. দৈনন্দিন বা ফাংশনাল কাজে বেশি অসুবিধা।
  4. আরও স্পষ্ট পোস্টেরিয়র ল্যাক্সিটি।

পোস্টেরিয়র ট্রান্সলেশন সাধারণত প্রায় ৬–১০ মিমি পর্যন্ত হতে পারে।

গ্রেড ৩ PCL টিয়ার

গ্রেড ৩ হলো হাই-গ্রেড ইনজুরি, যেখানে উল্লেখযোগ্য পোস্টেরিয়র ইনস্ট্যাবিলিটি থাকতে পারে।

এক্ষেত্রে দেখা যেতে পারে:

  1. গুরুতর PCL ড্যামেজ।
  2. উল্লেখযোগ্য পোস্টেরিয়র টিবিয়াল ট্রান্সলেশন।

পোস্টেরিয়র ট্রান্সলেশন প্রায় ১০ মিমির বেশি হলে হাই-গ্রেড ইনজুরির সম্ভাবনা বিবেচনা করা হয় এবং অন্য কোনো লিগামেন্ট ইনজুরি আছে কি না তাও মূল্যায়ন করা গুরুত্বপূর্ণ।

PCL-এর ক্লিনিক্যাল গ্রেডিং মূলত ল্যাক্সিটির ওপর নির্ভর করে, শুধু MRI-এর চেহারার ওপর নয়।

তবে শুধু ইনজুরির গ্রেড দেখেই চিকিৎসা নির্ধারণ করা হয় না। আরও বিবেচনা করা হয়:

  • শুধু PCL নাকি একাধিক লিগামেন্ট ইনজুরি

  • PLC ইনজুরি আছে কি না

  • দৈনন্দিন ও শারীরিক কার্যক্রমের চাহিদা
  • ব্যক্তিগত রিকভারির লক্ষ্য

PCL ইনজুরির সাধারণ উপসর্গ

ইনজুরির মেকানিজম ও তীব্রতার ওপর ভিত্তি করে উপসর্গ ভিন্ন হতে পারে।

সাধারণ উপসর্গের মধ্যে রয়েছে:

  1. হাঁটুর পেছনে বা গভীরে ব্যথা: ব্যথা হাঁটুর পেছনের দিকে অথবা জয়েন্টের গভীরে অনুভূত হতে পারে।
  2. হাঁটু ফুলে যাওয়া: ইনজুরির পর হাঁটুতে ফোলা দেখা দিতে পারে।
  3. হাঁটতে অসুবিধা: প্রাথমিক পর্যায়ে আক্রান্ত পায়ে ওজন নেওয়া ব্যথাযুক্ত বা অস্বস্তিকর হতে পারে।
  4. হাঁটু শক্ত হয়ে যাওয়া: ব্যথা ও ফোলার কারণে স্বাভাবিক মুভমেন্ট বাধাগ্রস্ত হতে পারে।
  5. রেঞ্জ অব মোশন কমে যাওয়া: হাঁটু ভাঁজ বা সোজা করার সক্ষমতা কমে যেতে পারে।
  6. হাঁটু অস্থির লাগা: বিশেষ করে বেশি শারীরিক চাপের কাজে হাঁটু অনির্ভরযোগ্য মনে হতে পারে।
  7. সিঁড়ি বা ঢাল বেয়ে নিচে নামতে অসুবিধা: হাঁটু ভাঁজ অবস্থায় লোড নেওয়ার সময় পোস্টেরিয়র ইনস্ট্যাবিলিটি বেশি অনুভূত হতে পারে।
  8. গতি কমাতে অসুবিধা: ডেসেলারেশনের সময় হাঁটু অস্বস্তিকর বা অস্থির লাগতে পারে।
  9. দৌড়ানোর সময় ব্যথা বা অস্থিরতা: সাধারণ হাঁটার সময় সমস্যা কম থাকলেও বেশি ডিমান্ডের অ্যাক্টিভিটিতে উপসর্গ প্রকাশ পেতে পারে।
  10. হাঁটুর ওপর আত্মবিশ্বাস কমে যাওয়া: প্রাথমিক ব্যথা কমে গেলেও খেলাধুলা বা বেশি শারীরিক কাজে রোগী দ্বিধা অনুভব করতে পারেন।

কিছু ACL ইনজুরির মতো শুধু PCL-এ সীমাবদ্ধ টিয়ারে সবসময় স্পষ্ট "পপ" শব্দ বা সঙ্গে সঙ্গে গুরুতর ইনস্ট্যাবিলিটি দেখা যায় না। এ কারণেই কিছু PCL ইনজুরি দেরিতে শনাক্ত হতে পারে।

PCL ইনজুরির সাধারণ কারণ

PCL ইনজুরির কিছু বিশেষ ও পরিচিত মেকানিজম রয়েছে।

  1. ড্যাশবোর্ড ইনজুরি:
    এটি PCL ইনজুরির অন্যতম ক্লাসিক মেকানিজম।

    গাড়ি দুর্ঘটনার সময়:

    • হাঁটু ভাঁজ অবস্থায় থাকে
    • শিনের উপরের সামনের অংশ ড্যাশবোর্ডে আঘাত করে
    • টিবিয়া ফিমারের তুলনায় পেছনের দিকে ধাক্কা খায়

    এর ফলে PCL-এর ওপর উল্লেখযোগ্য স্ট্রেস তৈরি হয়।

  2. ভাঁজ করা হাঁটুর ওপর সরাসরি পড়ে যাওয়া:
    হাঁটু ভাঁজ অবস্থায় শিনের সামনের অংশের ওপর সরাসরি পড়লে টিবিয়া পেছনের দিকে সরে যেতে পারে।

    এ ধরনের ঘটনা হতে পারে:

    • খেলাধুলার সময়
    • বিনোদনমূলক অ্যাক্টিভিটিতে
    • কর্মক্ষেত্রে দুর্ঘটনায়
    • সাধারণভাবে পড়ে গেলে
  3. হাইপারফ্লেক্সন ইনজুরি:
    হাঁটু অতিরিক্ত ভাঁজ হয়ে গেলে PCL-এর ওপর উল্লেখযোগ্য স্ট্রেস পড়তে পারে।
  4. হাইপারএক্সটেনশন ইনজুরি:
    হাঁটু জোরে অতিরিক্ত সোজা বা হাইপারএক্সটেনশনে চলে গেলে PCL ইনজুরি হতে পারে, বিশেষ করে একই সঙ্গে অন্য লিগামেন্টও ক্ষতিগ্রস্ত হলে।
  5. কন্টাক্ট স্পোর্টস:
    ফুটবল, রাগবি এবং অন্যান্য কন্টাক্ট বা কলিশন স্পোর্টসে হাঁটুর ওপর এমন ফোর্স তৈরি হতে পারে, যা PCL ইনজুরির কারণ হয়।
  6. খেলাধুলার সময় সংঘর্ষ:
    হাঁটু ভাঁজ করা অবস্থায় টিবিয়ার সামনের দিকে সরাসরি আঘাত লাগলে টিবিয়া পেছনের দিকে সরে গিয়ে PCL ক্ষতিগ্রস্ত হতে পারে।
  7. হাই-এনার্জি ট্রমা:
    বড় ধরনের দুর্ঘটনায় হতে পারে:
    • PCL ও PLC ইনজুরি
    • একাধিক লিগামেন্টের ইনজুরি
    • ফ্র্যাকচার বা অ্যাভালশন
    • নি ডিসলোকেশন-সম্পর্কিত ইনজুরি

    বড় ধরনের নি ট্রমায় কখনও কখনও আশেপাশের নার্ভ বা রক্তনালিও ক্ষতিগ্রস্ত হতে পারে। তাই এ ধরনের ইনজুরিতে দ্রুত ও বিস্তারিত মেডিকেল অ্যাসেসমেন্ট গুরুত্বপূর্ণ।

PCL ইনজুরির রিস্ক ফ্যাক্টর

PCL ইনজুরির একটি বড় অংশ ট্রমাটিক। তাই সব রিস্ক ফ্যাক্টর পরিবর্তনযোগ্য নয়।

সম্ভাব্য রিস্ক ফ্যাক্টর বা হাই-রিস্ক পরিস্থিতির মধ্যে রয়েছে:

  1. কন্টাক্ট স্পোর্টসে অংশগ্রহণ।
  2. ফুটবল, রাগবি এবং অন্যান্য কলিশন স্পোর্টস।
  3. মোটর ভেহিকেল অ্যাক্সিডেন্ট।
  4. এমন অ্যাক্টিভিটি যেখানে হাঁটুর ওপর সরাসরি পড়ে যাওয়ার ঝুঁকি থাকে।
  5. পূর্বে উল্লেখযোগ্য হাঁটুর ইনজুরি থাকা।

অনেক PCL ইনজুরি হঠাৎ ও অনির্দেশ্য আঘাতের কারণে হয়। তাই সব ইনজুরি শুধু স্ট্রেংথ বা মুভমেন্ট টেকনিক উন্নত করলেই প্রতিরোধ করা যেত, এমন ধারণা সঠিক নয়।

সঠিক রোগ নির্ণয়ের প্রথম ধাপ হলো কীভাবে ইনজুরি হয়েছে তা বোঝা।

ক্লিনিশিয়ান জানতে চাইতে পারেন:

  • হাঁটু ভাঁজ করা অবস্থায় শিনের সামনের দিকে আঘাত লেগেছিল কি না
  • রোগী সরাসরি হাঁটুর ওপর পড়েছিলেন কি না
  • হাঁটু অতিরিক্ত ফ্লেক্সনে গিয়েছিল কি না
  • হাঁটু হাইপারএক্সটেনশনে গিয়েছিল কি না
  • খেলাধুলার সময় সংঘর্ষ হয়েছিল কি না
  • হাই-এনার্জি ট্রমা ছিল কি না

ক্লিনিক্যাল অ্যাসেসমেন্টে থাকতে পারে:

  1. ব্যথার অবস্থান।
  2. হাঁটুর ফোলা।
  3. হাঁটুর রেঞ্জ অব মোশন।
  4. আক্রান্ত পায়ে ওজন নেওয়ার সক্ষমতা।
  5. হাঁটার প্যাটার্ন।
  6. হাঁটুর পোস্টেরিয়র স্ট্যাবিলিটি।
  7. কোয়াড্রিসেপস অ্যাক্টিভেশন ও মাসল স্ট্রেংথ।
  8. ফাংশনাল লিমিটেশন।
  9. রোটেশনাল স্ট্যাবিলিটি।
  10. অন্যান্য লিগামেন্ট বা মেনিস্কাস ইনজুরির সম্ভাবনা।

PCL ইনজুরির সঙ্গে অন্য লিগামেন্টও ইনজুরড হতে পারে। তাই শুধু PCL নয়, পুরো হাঁটু জয়েন্টের ক্লিনিক্যাল অ্যাসেসমেন্ট গুরুত্বপূর্ণ।

PCL ইনজুরির ক্লিনিক্যাল টেস্ট

পোস্টেরিয়র ইনস্ট্যাবিলিটি শনাক্ত করতে কয়েকটি ক্লিনিক্যাল টেস্ট ব্যবহার করা হয়।

পোস্টেরিয়র ড্রয়ার টেস্ট

পোস্টেরিয়র ড্রয়ার টেস্ট PCL ইনজুরি সন্দেহ হলে ব্যবহৃত অন্যতম গুরুত্বপূর্ণ ক্লিনিক্যাল টেস্ট।

সাধারণত হাঁটু প্রায় ৯০ ডিগ্রি ফ্লেক্সনে রেখে পরীক্ষক টিবিয়া ফিমারের তুলনায় কতটা পেছনের দিকে মুভ করছে তা মূল্যায়ন করেন।

অপর হাঁটুর তুলনায় টিবিয়া অতিরিক্ত পেছনের দিকে সরে গেলে PCL ইনসাফিসিয়েন্সির সম্ভাবনা থাকে।

হিপ ও হাঁটু ফ্লেক্সড অবস্থায় PCL দুর্বল বা ছিঁড়ে গেলে টিবিয়া স্বাভাবিকের তুলনায় পেছনের দিকে বসে যেতে পারে।

এই দৃশ্যমান বা স্পর্শ করে বোঝা পেছনের দিকে সরে যাওয়াকে পোস্টেরিয়র স্যাগ সাইন বলা হয়।

কোয়াড্রিসেপস অ্যাক্টিভ টেস্ট

কোয়াড্রিসেপস অ্যাক্টিভ টেস্টে দেখা হয় কোয়াড্রিসেপস অ্যাক্টিভ করার সময় পেছনের দিকে সরে থাকা টিবিয়া সামনে ফিরে আসে কি না।

এটি PCL ইনসাফিসিয়েন্সি শনাক্ত করতে অতিরিক্ত ক্লিনিক্যাল তথ্য দিতে পারে।

অন্যান্য ক্লিনিক্যাল টেস্ট

ইনজুরির মেকানিজম অনুযায়ী আরও মূল্যায়ন করা হতে পারে:

  1. ACL-এর ইন্টেগ্রিটি।
  2. MCL-এর ইন্টেগ্রিটি।
  3. LCL-এর ইন্টেগ্রিটি।

উল্লেখযোগ্য ট্রমার পর PCL ইনজুরি সন্দেহ হলে শুধু PCL-কে আলাদাভাবে মূল্যায়ন করা যথেষ্ট নয়।

PCL ইনজুরিতে MRI কি প্রয়োজন?

সন্দেহজনক PCL ইনজুরি মূল্যায়নের ক্ষেত্রে MRI একটি গুরুত্বপূর্ণ ইমেজিং পদ্ধতি।

ইনজুরির ইতিহাস এবং ক্লিনিক্যাল পরীক্ষা অত্যন্ত গুরুত্বপূর্ণ হলেও MRI PCL-এর ক্ষতি কনফার্ম করতে এবং মেনিস্কাস, কার্টিলেজ, হাড়, টেন্ডন বা অন্যান্য লিগামেন্টে ইনজুরি আছে কি না শনাক্ত করতে সাহায্য করতে পারে।

বিশেষ করে নতুন বা অ্যাকিউট PCL টিয়ার মূল্যায়নের ক্ষেত্রে MRI গুরুত্বপূর্ণ ভূমিকা রাখে।

নিচের পরিস্থিতিতে MRI বিশেষভাবে দরকার হতে পারে:

  1. উল্লেখযোগ্য পোস্টেরিয়র ইনস্ট্যাবিলিটি থাকলে।
  2. মাঝারি বা হাই-গ্রেড PCL ইনজুরি সন্দেহ হলে।
  3. একাধিক লিগামেন্ট ইনজুরি সন্দেহ হলে।
  4. PLC ইনজুরি সন্দেহ হলে।
  5. মেনিস্কাস বা কার্টিলেজ ইনজুরি সন্দেহ হলে।
  6. উপসর্গ দীর্ঘস্থায়ী হলে।
  7. রোগ নির্ণয় স্পষ্ট না হলে।
  8. সার্জারি বিবেচনা করা হলে।

এক্স-রে

বড় ধরনের ট্রমার পর এক্স-রে প্রয়োজন হতে পারে:

  • ফ্র্যাকচার শনাক্ত করতে
  • PCL অ্যাভালশন ফ্র্যাকচার দেখতে
  • অন্যান্য বোনি ইনজুরি শনাক্ত করতে

স্ট্রেস রেডিওগ্রাফ

কিছু নির্বাচিত রোগীর ক্ষেত্রে পোস্টেরিয়র স্ট্রেস রেডিওগ্রাফ ব্যবহার করে পোস্টেরিয়র টিবিয়াল ট্রান্সলেশনের মাত্রা পরিমাপ এবং অপর হাঁটুর সঙ্গে তুলনা করা যায়।

বিশেষ করে হাই-গ্রেড বা দীর্ঘদিনের ইনস্ট্যাবিলিটি এবং পোস্টেরিয়র ল্যাক্সিটির পরিমাণের ওপর চিকিৎসার সিদ্ধান্ত নির্ভর করলে এটি সহায়ক হতে পারে।

ইমেজিং রিপোর্ট সবসময় ক্লিনিক্যাল পরীক্ষা এবং রোগীর ফাংশনাল উপসর্গের সঙ্গে মিলিয়ে ব্যাখ্যা করা উচিত।

সব PCL ইনজুরিতেই কি সার্জারি প্রয়োজন?

না। অনেক ক্ষেত্রে শুধু PCL-এ সীমাবদ্ধ ইনজুরি সার্জারি ছাড়াই সফলভাবে ম্যানেজ করা সম্ভব।

লোয়ার-গ্রেড PCL ইনজুরির ক্ষেত্রে সাধারণত নন-অপারেটিভ চিকিৎসা বিবেচনা করা হয়। কিছু নির্বাচিত হাই-গ্রেড ইনজুরিতেও হাঁটুর ফাংশনাল স্ট্যাবিলিটি এবং রোগীর অ্যাক্টিভিটি ডিমান্ড অনুযায়ী নন-সার্জিক্যাল ম্যানেজমেন্ট উপযুক্ত হতে পারে।

স্ট্রাকচার্ড নন-অপারেটিভ রিহ্যাবিলিটেশনের মাধ্যমে অনেক রোগী ভালো ফাংশন ফিরে পেতে এবং খেলাধুলায় ফিরতে পারেন। তবে কিছু ক্ষেত্রে কিছুটা পোস্টেরিয়র ল্যাক্সিটি থেকে যেতে পারে এবং দীর্ঘমেয়াদে জয়েন্টে লোড পড়ার ধরন পরিবর্তিত হতে পারে।

নন-সার্জিক্যাল চিকিৎসায় যা থাকতে পারে

প্রাথমিক PCL রিহ্যাবিলিটেশনের অন্যতম গুরুত্বপূর্ণ নীতি হলো লিগামেন্ট হিলিংয়ের সময় অতিরিক্ত পোস্টেরিয়র টিবিয়াল ট্রান্সলেশন কমিয়ে রাখা।

কখন PCL সার্জারি বিবেচনা করা হতে পারে?

শুধু MRI রিপোর্টে PCL টিয়ার দেখা গেছে বলেই সার্জারি করতে হবে, এমন নয়।

নির্বাচিত কিছু ক্ষেত্রে PCL রিকনস্ট্রাকশন বা অন্যান্য সার্জিক্যাল চিকিৎসা বিবেচনা করা হতে পারে, যেমন:

  1. উল্লেখযোগ্য পোস্টেরিয়র ইনস্ট্যাবিলিটিসহ হাই-গ্রেড PCL ইনজুরি।
  2. স্ট্রাকচার্ড রিহ্যাবিলিটেশনের পরও দীর্ঘস্থায়ী ও উপসর্গযুক্ত ইনস্ট্যাবিলিটি।
  3. একাধিক লিগামেন্ট ইনজুরি।

  6. ডিসপ্লেসড PCL অ্যাভালশন ইনজুরি।

চিকিৎসার সিদ্ধান্ত পোস্টেরিয়র ইনস্ট্যাবিলিটি, অন্য কোনো ইনজুরি, রোগীর দৈনন্দিন ও খেলাধুলার চাহিদা এবং ব্যক্তিগত লক্ষ্য অনুযায়ী নেওয়া উচিত।

PCL ইনজুরিতে ফিজিওথেরাপির ভূমিকা

নন-অপারেটিভ PCL চিকিৎসা এবং PCL রিকনস্ট্রাকশনের পর রিহ্যাবিলিটেশন, উভয় ক্ষেত্রেই ফিজিওথেরাপির গুরুত্বপূর্ণ ভূমিকা রয়েছে।

PCL-এর বায়োমেকানিক্স ACL-এর থেকে আলাদা। তাই ACL-এর জন্য ব্যবহৃত একই রিহ্যাবিলিটেশন প্রোগ্রাম সরাসরি PCL ইনজুরিতে প্রয়োগ করা উচিত নয়। PCL-এর ওপর কী ধরনের ফোর্স কাজ করে, তা বিবেচনা করে রিহ্যাবিলিটেশন পরিকল্পনা করা প্রয়োজন।

  1. ব্যথা ও ফোলা কমানো:
    প্রাথমিক পর্যায়ে রিহ্যাবিলিটেশনের লক্ষ্য হলো:
    1. ব্যথা নিয়ন্ত্রণ।
    2. জয়েন্টের ফোলা কমানো।
    3. ইনজুরড লিগামেন্টকে সুরক্ষা দেওয়া।
    4. নিরাপদ মুভমেন্ট বজায় রাখা।

    হাঁটু ফুলে থাকলে কোয়াড্রিসেপস অ্যাক্টিভেশন কমে যেতে পারে। তাই প্রাথমিক পর্যায়ে মাসল কন্ট্রোল পুনরুদ্ধার গুরুত্বপূর্ণ।

  2. হাঁটুর রেঞ্জ অব মোশন পুনরুদ্ধার:
    হিলিং PCL-এর ওপর অপ্রয়োজনীয় স্ট্রেস না দিয়ে ধীরে ধীরে হাঁটুর মুভমেন্ট ফিরিয়ে আনা উচিত।

    সাধারণ লক্ষ্য হতে পারে:

    • ফুল নি এক্সটেনশন ফিরিয়ে আনা
    • ধীরে ধীরে নি ফ্লেক্সন বাড়ানো
    • অতিরিক্ত স্টিফনেস প্রতিরোধ করা

    যদি অতিরিক্ত বা খুব দ্রুত ফ্লেক্সন পোস্টেরিয়র টিবিয়াল স্ট্রেস বাড়ায়, তাহলে প্রাথমিক পর্যায়ে খুব দ্রুত বেশি ফ্লেক্সন প্রগ্রেস করা উচিত নয়।

  3. কোয়াড্রিসেপস স্ট্রেংথ পুনরুদ্ধার:
    PCL রিহ্যাবিলিটেশনে কোয়াড্রিসেপস স্ট্রেংথেনিং বিশেষভাবে গুরুত্বপূর্ণ।

    শক্তিশালী কোয়াড্রিসেপস টিবিয়ার কন্ট্রোল উন্নত করতে এবং পোস্টেরিয়র টিবিয়াল স্যাগের বিপরীতে কাজ করতে সাহায্য করে।

    প্রগ্রেসিভ কোয়াড্রিসেপস স্ট্রেংথেনিং সাহায্য করতে পারে:

    1. হাঁটুর কন্ট্রোল উন্নত করতে।
    2. হাঁটার সক্ষমতা ফিরিয়ে আনতে।

  4. হিপ ও লোয়ার লিম্ব শক্তিশালী করা:
    PCL রিহ্যাবিলিটেশনে শুধু হাঁটুকে নয়, পুরো লোয়ার লিম্ব কাইনেটিক চেইন বিবেচনা করা উচিত।

    PCL ইনজুরিতে হ্যামস্ট্রিং প্রগ্রেশন আলাদা কেন?

    হ্যামস্ট্রিং কনট্রাকশন টিবিয়াকে পেছনের দিকে টানতে পারে।

    এই কারণে PCL রিহ্যাবিলিটেশনের প্রাথমিক পর্যায়ে ভারী বা রেজিস্টেড হ্যামস্ট্রিং লোডিং সীমিত রাখা হতে পারে। টিস্যু হিলিং, হাঁটুর স্ট্যাবিলিটি এবং ক্লিনিক্যাল নির্দেশনা অনুযায়ী হ্যামস্ট্রিং এক্সারসাইজ ধীরে ধীরে প্রগ্রেস করা হয়।

    এটি PCL রিহ্যাবিলিটেশন এবং অনেক অন্যান্য নি রিহ্যাবিলিটেশন প্রোগ্রামের মধ্যে একটি গুরুত্বপূর্ণ পার্থক্য।

    বিশেষ করে সিঁড়ি দিয়ে নিচে নামা, ডেসেলারেশন এবং লোডেড নি ফ্লেক্সন-এর সময় বেশি মনোযোগ প্রয়োজন হতে পারে, কারণ এসব অ্যাক্টিভিটিতে পোস্টেরিয়র ইনস্ট্যাবিলিটির কারণে থাকা ফাংশনাল ঘাটতি বেশি প্রকাশ পেতে পারে।

  7. নিরাপদ রিটার্ন-টু-স্পোর্টের প্রস্তুতি:
    খেলাধুলায় ফেরার সিদ্ধান্ত শুধু ইনজুরির পর কত সপ্তাহ বা মাস হয়েছে তার ওপর নির্ভর করা উচিত নয়।

    রিটার্ন-টু-স্পোর্ট প্রগ্রেশন শুধু সময়ের ওপর নয়, স্ট্রেংথ ও ফাংশনাল ক্রাইটেরিয়ার ওপর ভিত্তি করে হওয়া উচিত।

PCL রিহ্যাবিলিটেশনের স্টেজ

প্রতিটি PCL ইনজুরির জন্য ব্যক্তিভিত্তিক রিহ্যাবিলিটেশন প্ল্যান প্রয়োজন। তবে সাধারণভাবে রিকভারিকে কয়েকটি স্টেজে ভাগ করে বোঝানো যায়।

স্টেজ ১: হাঁটুকে সুরক্ষা দেওয়া ও বেসিক ফাংশন ফিরিয়ে আনা

প্রাথমিক পর্যায়ে ইনজুরড PCL-কে সুরক্ষা দেওয়ার পাশাপাশি নিরাপদ মুভমেন্ট ও মাসল অ্যাক্টিভেশন বজায় রাখার ওপর গুরুত্ব দেওয়া হয়।

লক্ষ্য হতে পারে:

  1. ব্যথা ও ফোলা কমানো।
  2. হিলিং PCL-কে সুরক্ষা দেওয়া।
  3. অতিরিক্ত পোস্টেরিয়র টিবিয়াল স্যাগ সীমিত করা।

  5. ধীরে ধীরে নি ফ্লেক্সন ফিরিয়ে আনা।

  7. নিরাপদভাবে ওয়েট-বেয়ারিং প্রগ্রেস করা।
  8. উপযুক্ত হাঁটার প্যাটার্ন পুনরুদ্ধার করা।

নির্বাচিত কিছু ক্ষেত্রে পোস্টেরিয়র টিবিয়াল ডিসপ্লেসমেন্ট নিয়ন্ত্রণে সাহায্য করার জন্য PCL-স্পেসিফিক ব্রেস বিবেচনা করা যেতে পারে।

ডায়নামিক PCL ব্রেস টিবিয়াকে সামনের দিকে সাপোর্ট দিয়ে পোস্টেরিয়র স্যাগ কমাতে সাহায্য করার জন্য ব্যবহার করা হতে পারে।

স্টেজ ২: স্ট্রেংথ ও মুভমেন্ট পুনর্গঠন

উপসর্গ কমে আসা এবং ইনজুরড লিগামেন্ট ধীরে ধীরে বেশি লোড সহ্য করতে পারার সঙ্গে সঙ্গে রিহ্যাবিলিটেশন আরও অ্যাক্টিভ হয়।

এই পর্যায়ে থাকতে পারে:

  1. প্রগ্রেসিভ কোয়াড্রিসেপস স্ট্রেংথেনিং।
  2. হিপ ও গ্লুটিয়াল স্ট্রেংথেনিং।

স্টেজ ৩: অ্যাডভান্সড স্ট্রেংথ ও ডায়নামিক কন্ট্রোল

বেসিক স্ট্রেংথ ও মোবিলিটি উন্নত হওয়ার পর রিহ্যাবিলিটেশন প্রগ্রেস হতে পারে:

  1. হাই-লেভেল কোয়াড্রিসেপস স্ট্রেংথেনিং।
  2. সিঙ্গেল-লেগ স্ট্রেংথেনিং।
  3. ডায়নামিক ব্যালেন্স।
  4. ফাংশনাল ক্লোজড-চেইন লোডিং।
  5. ল্যান্ডিংয়ের প্রস্তুতি।
  6. রানিংয়ের প্রস্তুতি।
  7. প্রগ্রেসিভ নিউরোমাসকুলার ট্রেনিং।

হাই-লেভেল অ্যাক্টিভিটিতে ফিরতে ইচ্ছুক রোগীদের ক্ষেত্রে ধীরে ধীরে যোগ করা হতে পারে:

  1. রানিং।
  2. অ্যাক্সেলারেশন।
  3. ডেসেলারেশন।
  4. চেঞ্জ-অব-ডিরেকশন ড্রিল।
  5. জাম্পিং।
  6. ল্যান্ডিং।
  7. অ্যাজিলিটি ট্রেনিং।
  8. স্পোর্টস-স্পেসিফিক মুভমেন্ট প্যাটার্ন।

এই পর্যায়ে ডেসেলারেশন এবং নিয়ন্ত্রিত নি ফ্লেক্সন বিশেষভাবে গুরুত্বপূর্ণ, কারণ এগুলো হাঁটুর ওপর উল্লেখযোগ্য ডিমান্ড তৈরি করে।
