MCL হাঁটুর ভেতরের দিকে অবস্থিত এবং হাঁটুর স্ট্যাবিলিটি বজায় রাখতে গুরুত্বপূর্ণ ভূমিকা রাখে। বিশেষ করে হাঁটু অতিরিক্ত ভেতরের দিকে চলে যাওয়া বা ভ্যালগাস স্ট্রেস প্রতিরোধে এটি গুরুত্বপূর্ণ। MCL অতিরিক্ত স্ট্রেচ বা ছিঁড়ে গেলে হাঁটুর ভেতরের দিকে ব্যথা, টেন্ডারনেস, ফোলা, হাঁটতে অসুবিধা বা হাঁটু অস্থির লাগার মতো উপসর্গ দেখা দিতে পারে।

MCL ইনজুরি হালকা লিগামেন্ট স্প্রেইন থেকে সম্পূর্ণ টিয়ার পর্যন্ত হতে পারে। ভালো দিক হলো, অনেক isolated MCL ইনজুরি সার্জারি ছাড়াই সফলভাবে ম্যানেজ করা সম্ভব, কারণ MCL-এর হিলিং পটেনশিয়াল তুলনামূলকভাবে ভালো। তাই উপযুক্ত প্রোটেকশন, প্রগ্রেসিভ এক্সারসাইজ এবং স্ট্রাকচার্ড ফিজিওথেরাপি অনেক রোগীর রিকভারিতে গুরুত্বপূর্ণ ভূমিকা রাখে।

তবে চিকিৎসা নির্ভর করে ইনজুরির তীব্রতা ও অবস্থান, হাঁটুর স্ট্যাবিলিটি, ACL বা মেনিস্কাসসহ অন্য কোনো স্ট্রাকচারের ইনজুরি, রোগীর অ্যাক্টিভিটি লেভেল এবং রিকভারি গোলের ওপর। এই গাইডে MCL কী, MCL ইনজুরির উপসর্গ ও কারণ, কীভাবে রোগ নির্ণয় করা হয়, সম্ভাব্য চিকিৎসা পদ্ধতি এবং প্রাথমিক পর্যায় থেকে দৈনন্দিন কাজ বা খেলাধুলায় ফিরে যাওয়া পর্যন্ত ফিজিওথেরাপি রিহ্যাবিলিটেশনের ভূমিকা আলোচনা করা হয়েছে।

MCL কী?

Medial Collateral Ligament বা MCL হাঁটুর ভেতরের দিকে অবস্থিত প্রধান স্ট্যাবিলাইজিং স্ট্রাকচারগুলোর একটি।

এটি উরুর হাড় বা ফিমারকে পায়ের নিচের অংশের টিবিয়ার সঙ্গে যুক্ত করে। অন্যান্য লিগামেন্ট, জয়েন্ট ক্যাপসুল, মেনিস্কাস এবং আশেপাশের মাসলের সঙ্গে সমন্বিতভাবে MCL দাঁড়ানো, হাঁটা, দৌড়ানো এবং খেলাধুলার সময় হাঁটুকে স্ট্যাবিলিটি দিতে সাহায্য করে।

MCL-এর অন্যতম প্রধান কাজ হলো অতিরিক্ত ভ্যালগাস স্ট্রেস প্রতিরোধ করা। অর্থাৎ এমন ফোর্স প্রতিরোধ করা, যেখানে হাঁটু পায়ের নিচের অংশের তুলনায় অতিরিক্ত ভেতরের দিকে চলে যায়। হাঁটুর রোটেশনাল কন্ট্রোলেও MCL ভূমিকা রাখে।

মিডিয়াল লিগামেন্ট কমপ্লেক্সে সুপারফিশিয়াল ও গভীর স্ট্রাকচার রয়েছে। Superficial MCL ভ্যালগাস স্ট্রেস প্রতিরোধে প্রধান ভূমিকা রাখে, আর গভীর মিডিয়াল স্ট্রাকচারগুলো জয়েন্ট ও রোটেশনাল স্ট্যাবিলিটিতে অতিরিক্ত সহায়তা করে। রোগীর জন্য গুরুত্বপূর্ণ বিষয় হলো, MCL ক্ষতিগ্রস্ত হলে হাঁটু পাশ থেকে আসা ফোর্স এবং রোটেশনাল লোড সহ্য করার ক্ষমতা কমে যেতে পারে।

MCL ইনজুরি কী?

অতিরিক্ত ফোর্সের কারণে Medial Collateral Ligament স্ট্রেচ বা ছিঁড়ে গেলে তাকে MCL ইনজুরি বলা হয়।

MCL ইনজুরির একটি ক্লাসিক মেকানিজম হলো হাঁটুর বাইরের দিকে সরাসরি আঘাত, যার ফলে হাঁটু ভেতরের দিকে ধাক্কা খায়। কন্টাক্ট স্পোর্টসে অন্য খেলোয়াড় হাঁটুর বাইরের দিকে আঘাত করলে এ ধরনের ইনজুরি হতে পারে।

তবে সরাসরি সংঘর্ষ ছাড়াও MCL ইনজুরি হতে পারে। পা মাটিতে স্থির থাকা অবস্থায় হাঁটু টুইস্ট হওয়া, দিক পরিবর্তনের সময় কন্ট্রোল হারানো অথবা ভুলভাবে ল্যান্ড করার কারণে পর্যাপ্ত ভ্যালগাস ও রোটেশনাল ফোর্স তৈরি হয়ে MCL ক্ষতিগ্রস্ত হতে পারে।

কিছু ক্ষেত্রে শুধু MCL ইনজুরি হয়। আবার বেশি শক্তিশালী ট্রমায় ACL, মেনিস্কাস, পোস্টেরোমিডিয়াল স্ট্রাকচার, কার্টিলেজ বা হাঁটুর অন্যান্য লিগামেন্টও একই সঙ্গে ইনজুরড হতে পারে। বিশেষ করে বেশি রোটেশনাল মেকানিজম থাকলে isolated MCL স্প্রেইনের পাশাপাশি combined ligament injury-এর সম্ভাবনাও বিবেচনা করা জরুরি।

এ কারণেই শুধু হাঁটুর ভেতরের দিকে ব্যথা হচ্ছে বলে MCL ইনজুরিকে হালকাভাবে নেওয়া উচিত নয়। সঠিক ক্লিনিক্যাল অ্যাসেসমেন্ট গুরুত্বপূর্ণ।

MCL ইনজুরির ধরন ও গ্রেড

লিগামেন্টের ক্ষতির মাত্রা এবং হাঁটুর ইনস্ট্যাবিলিটি অনুযায়ী MCL ইনজুরি সাধারণত বিভিন্ন গ্রেডে ভাগ করা হয়।

  1. গ্রেড ১ MCL স্প্রেইন:
    গ্রেড ১ হলো তুলনামূলক হালকা স্প্রেইন। লিগামেন্টের অল্প কিছু ফাইবার স্ট্রেচ বা ক্ষতিগ্রস্ত হতে পারে, তবে সামগ্রিকভাবে লিগামেন্টের স্ট্রাকচার অক্ষত থাকে।
    রোগীর মধ্যে দেখা যেতে পারে:
    1. MCL-এর বরাবর নির্দিষ্ট স্থানে টেন্ডারনেস।
    2. হালকা ব্যথা।
    3. সামান্য ফোলা।
    4. উল্লেখযোগ্য ভ্যালগাস ইনস্ট্যাবিলিটি না থাকা।
  2. গ্রেড ২ MCL স্প্রেইন:
    গ্রেড ২ ইনজুরিতে লিগামেন্ট আংশিকভাবে ছিঁড়ে যায় বা পার্শিয়াল টিয়ার হয়।
    রোগীর মধ্যে দেখা যেতে পারে:
    1. হাঁটুর ভেতরের দিকে তুলনামূলক বেশি ব্যথা।
    2. স্থানীয় ফোলা বা ব্রুইজিং।
    3. হাঁটা বা পাশের দিকে মুভমেন্টের সময় ব্যথা।
    4. মিডিয়াল দিকে কিছুটা অতিরিক্ত ল্যাক্সিটি।
  3. গ্রেড ৩ MCL টিয়ার:
    গ্রেড ৩ ইনজুরিতে লিগামেন্ট সম্পূর্ণভাবে ছিঁড়ে যায় বা ডিসরাপ্ট হয়।
    এক্ষেত্রে দেখা যেতে পারে:
    1. উল্লেখযোগ্য লিগামেন্ট ড্যামেজ।
    2. মিডিয়াল হাঁটুর ইনস্ট্যাবিলিটি।
    3. ভ্যালগাস স্ট্রেস সহ্য করতে অসুবিধা।
    4. বেশি ফাংশনাল লিমিটেশন।
    5. জটিল ক্ষেত্রে হাঁটুর অন্যান্য স্ট্যাবিলাইজিং স্ট্রাকচারেও ইনজুরি।
    ক্লিনিক্যাল গ্রেডিং সিস্টেমে কিছুটা পার্থক্য থাকতে পারে। তবে সম্পূর্ণ MCL ইনজুরিতে সাধারণত পার্শিয়াল ইনজুরির তুলনায় বেশি ভ্যালগাস ল্যাক্সিটি দেখা যায়। হাঁটু শুধু ফ্লেক্সনে থাকা অবস্থায় অস্থির কি না, নাকি ফুল এক্সটেনশনেও অস্থিরতা আছে, সেটিও ইনজুরির বিস্তার বুঝতে গুরুত্বপূর্ণ।
    শুধু গ্রেড দেখেই চিকিৎসা নির্ধারণ করা হয় না। টিয়ারের অবস্থান, ফাংশনাল স্ট্যাবিলিটি, ACL বা মেনিস্কাস ইনজুরি, রোগীর দৈনন্দিন ও স্পোর্টস ডিমান্ড এবং ব্যক্তিগত রিকভারি গোলও বিবেচনা করা হয়।

MCL ইনজুরির সাধারণ উপসর্গ

ইনজুরির তীব্রতা এবং অন্য কোনো স্ট্রাকচার ইনজুরড হয়েছে কি না তার ওপর উপসর্গ ভিন্ন হতে পারে।

সাধারণ উপসর্গের মধ্যে রয়েছে:

  1. হাঁটুর ভেতরের দিকে ব্যথা: এটি MCL ইনজুরির সবচেয়ে পরিচিত উপসর্গগুলোর একটি।
  2. লিগামেন্টের বরাবর টেন্ডারনেস: ইনজুরড অংশে চাপ দিলে ব্যথা অনুভূত হতে পারে।
  3. ফোলা: হাঁটুর মিডিয়াল বা ভেতরের দিকে স্থানীয় ফোলা দেখা দিতে পারে।
  4. হাঁটা বা ওয়েট-বেয়ারিংয়ের সময় ব্যথা: তুলনামূলক গুরুতর ইনজুরিতে স্বাভাবিকভাবে হাঁটাও কষ্টকর হতে পারে।
  5. সাইড-টু-সাইড স্ট্রেসে ব্যথা: হাঁটুর ওপর ভ্যালগাস স্ট্রেস তৈরি করে এমন মুভমেন্টে ব্যথা বাড়তে পারে।
  6. হাঁটুর মুভমেন্ট কমে যাওয়া: ব্যথা ও ফোলার কারণে হাঁটু ভাঁজ বা সোজা করতে অস্বস্তি হতে পারে।
  7. হাঁটুর ইনস্ট্যাবিলিটি: মাঝারি বা গুরুতর ইনজুরিতে হাঁটু কম স্ট্যাবল মনে হতে পারে।
  8. কাটিং বা পিভটিংয়ে সমস্যা: বিশেষ করে অ্যাথলেটরা দ্রুত দিক পরিবর্তন বা ল্যাটারাল মুভমেন্টের সময় সমস্যা অনুভব করতে পারেন।

Acute isolated MCL ইনজুরিতে হাঁটুর ভেতরের দিকে স্থানীয় টেন্ডারনেস ও ফোলা সাধারণ। তবে উল্লেখযোগ্য পুরো জয়েন্ট ফুলে যাওয়া বা বেশি ইনস্ট্যাবিলিটি থাকলে intra-articular অথবা multi-ligament injury-এর সম্ভাবনাও বিবেচনা করা দরকার।

MCL ইনজুরির সাধারণ কারণ

হাঁটুর ওপর অতিরিক্ত ভ্যালগাস স্ট্রেস তৈরি হলে MCL ইনজুরি বেশি হয়।

সাধারণ ইনজুরি মেকানিজমের মধ্যে রয়েছে:

  1. হাঁটুর বাইরের দিকে সরাসরি আঘাত: সংঘর্ষের কারণে হাঁটু ভেতরের দিকে ধাক্কা খায়।
  2. ফুটবল ট্যাকল বা খেলোয়াড়ের সঙ্গে সংঘর্ষ: কন্টাক্ট স্পোর্টসে MCL ইনজুরি একটি পরিচিত সমস্যা।
  3. পা মাটিতে স্থির থাকা অবস্থায় হাঁটু টুইস্ট হওয়া: রোটেশন ও ভ্যালগাস স্ট্রেস একসঙ্গে MCL-এর ওপর চাপ সৃষ্টি করতে পারে।
  4. ভুলভাবে ল্যান্ড করা: জাম্পের পর সঠিক কন্ট্রোল না থাকলে মিডিয়াল হাঁটুর ওপর অতিরিক্ত চাপ পড়তে পারে।
  5. হঠাৎ দিক পরিবর্তন: কাটিং মুভমেন্টের সময় ভ্যালগাস ও রোটেশনাল লোড তৈরি হতে পারে।
  6. স্কিইং ইনজুরি: স্কি স্থির থাকা অবস্থায় পড়ে যাওয়া বা হাঁটু টুইস্ট হওয়ার কারণে MCL ইনজুরি হতে পারে।
  7. পড়ে যাওয়া বা দুর্ঘটনাজনিত ট্রমা: বড় ধরনের আঘাতে MCL-এর সঙ্গে অন্যান্য হাঁটুর স্ট্রাকচারও ইনজুরড হতে পারে।

সরাসরি কন্টাক্ট MCL ইনজুরির একটি বিশেষভাবে পরিচিত মেকানিজম, তবে নন-কন্টাক্ট ইনজুরিও হতে পারে।

MCL ইনজুরির রিস্ক ফ্যাক্টর

MCL ইনজুরি সাধারণত কোনো একটি কারণের ফল নয়। হাই-রিস্ক মুভমেন্টের এক্সপোজার, ব্যক্তিগত ফ্যাক্টর এবং পরিবেশগত অবস্থার সমন্বয়ে ঝুঁকি বাড়তে পারে।

সম্ভাব্য রিস্ক ফ্যাক্টরের মধ্যে রয়েছে:

  1. নিয়মিত কন্টাক্ট স্পোর্টসে অংশগ্রহণ।
  2. ফুটবল, রাগবি, বাস্কেটবল বা অন্যান্য মাল্টিডিরেকশনাল স্পোর্টস।
  3. স্কিইং।
  4. পূর্বে হাঁটুর লিগামেন্ট ইনজুরি থাকা।
  5. লোয়ার লিম্বের স্ট্রেংথ কম থাকা।
  6. দুর্বল নিউরোমাসকুলার কন্ট্রোল।
  7. ভুল ল্যান্ডিং বা কাটিং মেকানিক্স।
  8. বারবার খেলোয়াড়ের সংস্পর্শ বা অনির্দেশ্য সাইড-টু-সাইড ফোর্সের এক্সপোজার।

তবে সব MCL ইনজুরি প্রতিরোধ করা সম্ভব নয়। বিশেষ করে হঠাৎ এবং অনির্দেশ্য সংঘর্ষের কারণে হওয়া ট্রমাটিক MCL ইনজুরি অনেক ক্ষেত্রে প্রতিরোধ করা কঠিন।

MCL ইনজুরি কীভাবে নির্ণয় করা হয়?

MCL ইনজুরি নির্ণয়ের শুরুতেই কীভাবে ইনজুরি হয়েছে তা বিস্তারিতভাবে জানা হয়।

ক্লিনিশিয়ান জানতে চাইতে পারেন হাঁটুর বাইরের দিকে সরাসরি আঘাত লেগেছিল কি না, পা মাটিতে রেখে হাঁটু টুইস্ট হয়েছিল কি না, ল্যান্ডিংয়ের সময় হাঁটু ভেতরের দিকে চলে গিয়েছিল কি না অথবা অন্য কোনো ট্রমাটিক মেকানিজম ছিল কি না।

ক্লিনিক্যাল অ্যাসেসমেন্টের মধ্যে থাকতে পারে:

  1. ব্যথার সঠিক অবস্থান।
  2. ফোলা বা ব্রুইজিং।
  3. হাঁটুর ভেতরের দিকে টেন্ডারনেস।
  4. হাঁটুর রেঞ্জ অব মোশন।
  5. আক্রান্ত পায়ে ওজন নেওয়ার সক্ষমতা।
  6. হাঁটার প্যাটার্ন।
  7. মিডিয়াল হাঁটুর স্ট্যাবিলিটি।
  8. মাসল অ্যাক্টিভেশন ও স্ট্রেংথ।
  9. ফাংশনাল লিমিটেশন।
  10. ACL, PCL, মেনিস্কাস বা অন্য কোনো লিগামেন্ট ইনজুরির সম্ভাবনা।

পরীক্ষা শুধু MCL-এর মধ্যে সীমাবদ্ধ রাখা উচিত নয়, কারণ উল্লেখযোগ্য ভ্যালগাস বা রোটেশনাল ট্রমায় হাঁটুর একাধিক স্ট্রাকচার একই সঙ্গে ইনজুরড হতে পারে।

MCL ইনজুরির ক্লিনিক্যাল টেস্ট

MCL ইনজুরি সন্দেহ হলে ভ্যালগাস স্ট্রেস টেস্ট অন্যতম গুরুত্বপূর্ণ ক্লিনিক্যাল টেস্ট।

টেস্টটি সাধারণত হাঁটু প্রায় ৩০ ডিগ্রি ফ্লেক্সনে রেখে এবং আবার প্রায় ফুল এক্সটেনশনে করা হতে পারে।

৩০ ডিগ্রি ফ্লেক্সনে অতিরিক্ত মিডিয়াল ওপেনিং বা ল্যাক্সিটি বিশেষ করে superficial MCL ইনজুরির ইঙ্গিত দিতে পারে।

অন্যদিকে ফুল এক্সটেনশনেও যদি উল্লেখযোগ্য ভ্যালগাস ইনস্ট্যাবিলিটি থাকে, তাহলে অতিরিক্ত মিডিয়াল স্ট্যাবিলাইজার এবং সম্ভাব্যভাবে এক বা উভয় ক্রুসিয়েট লিগামেন্ট ইনজুরির বিষয়ও বিবেচনা করতে হয়।

ইনজুরির মেকানিজম অনুযায়ী আরও মূল্যায়ন করা হতে পারে:

  1. ACL ইন্টেগ্রিটি।
  2. PCL ইন্টেগ্রিটি।
  3. মেনিস্কাস ইনজুরি।
  4. রোটেশনাল স্ট্যাবিলিটি।
  5. অন্যান্য লিগামেন্ট স্ট্রাকচার।

ক্লিনিক্যাল স্পেশাল টেস্ট সেলফ-ডায়াগনোসিসের জন্য নয়। প্রশিক্ষিত স্বাস্থ্য পেশাজীবীর মাধ্যমে সঠিকভাবে টেস্ট করা এবং ফলাফল ব্যাখ্যা করা উচিত।

MCL ইনজুরিতে এমআরআই কি প্রয়োজন?

সব MCL ইনজুরিতেই এমআরআই করার প্রয়োজন হয় না।

অনেক isolated গ্রেড ১ ও গ্রেড ২ MCL ইনজুরি ইনজুরির ইতিহাস এবং ক্লিনিক্যাল অ্যাসেসমেন্টের মাধ্যমেই শনাক্ত করা যায়।

নিচের পরিস্থিতিতে এমআরআই বিশেষভাবে উপকারী হতে পারে:

  1. ইনজুরি গুরুতর মনে হলে।
  2. গ্রেড ৩ MCL ইনজুরি সন্দেহ হলে।
  3. উল্লেখযোগ্য ইনস্ট্যাবিলিটি থাকলে।
  4. একাধিক লিগামেন্ট ইনজুরি সন্দেহ হলে।
  5. মেনিস্কাস বা কার্টিলেজ ইনজুরি সন্দেহ হলে।
  6. টিয়ারের অবস্থান বা বিস্তার স্পষ্ট করা দরকার হলে।
  7. রোগীর উপসর্গ ও ক্লিনিক্যাল ফাইন্ডিংসের মধ্যে মিল না থাকলে।
  8. সার্জিক্যাল প্ল্যানিং বিবেচনা করা হলে।

এমআরআই MCL টিয়ারের অবস্থান এবং একই সঙ্গে সফট টিস্যু, অন্যান্য লিগামেন্ট, মেনিস্কাস বা বোন ইনজুরি শনাক্ত করতে সাহায্য করতে পারে।

এক্স-রেতে MCL টিয়ার সরাসরি দেখা যায় না। তবে বড় ধরনের ট্রমার পর ফ্র্যাকচার, অ্যাভালশন ইনজুরি বা অন্য কোনো বোনি প্যাথলজি বাদ দিতে এক্স-রে প্রয়োজন হতে পারে।

ইমেজিং রিপোর্ট সবসময় ক্লিনিক্যাল অ্যাসেসমেন্টের সঙ্গে মিলিয়ে ব্যাখ্যা করা উচিত।

সব MCL ইনজুরিতেই কি সার্জারি প্রয়োজন?

না। অধিকাংশ isolated MCL ইনজুরি সার্জারি ছাড়াই ম্যানেজ করা হয়।

গ্রেড ১ ও গ্রেড ২ MCL ইনজুরি সাধারণত নন-অপারেটিভভাবে চিকিৎসা করা হয়। অনেক isolated গ্রেড ৩ MCL টিয়ারও উপযুক্ত প্রোটেকশন, প্রগ্রেসিভ রিহ্যাবিলিটেশন এবং হাঁটুর স্ট্যাবিলিটির সঠিক মূল্যায়নের মাধ্যমে ভালোভাবে হিল করতে পারে।

MCL-এর তুলনামূলক ভালো হিলিং ক্যাপাসিটি রয়েছে। তাই অধিকাংশ isolated ইনজুরিতে নন-অপারেটিভ চিকিৎসাই প্রধান ম্যানেজমেন্ট পদ্ধতি।

নন-সার্জিক্যাল চিকিৎসায় যা থাকতে পারে

  1. অ্যাক্টিভিটি মডিফিকেশন।
  2. হিলিং লিগামেন্টকে উপযুক্ত প্রোটেকশন দেওয়া।
  3. ক্লিনিক্যালি প্রয়োজন হলে ব্রেস ব্যবহার।
  4. ব্যথা ও ফোলা নিয়ন্ত্রণ।
  5. উপযুক্তভাবে প্রগ্রেসিভ ওয়েট-বেয়ারিং।
  6. হাঁটুর রেঞ্জ অব মোশন পুনরুদ্ধার।
  7. কোয়াড্রিসেপস স্ট্রেংথেনিং।
  8. হ্যামস্ট্রিং স্ট্রেংথেনিং।
  9. হিপ ও লোয়ার লিম্ব স্ট্রেংথেনিং।
  10. ব্যালেন্স ও প্রোপ্রিওসেপশন ট্রেনিং।
  11. ফাংশনাল রিহ্যাবিলিটেশন।
  12. ধীরে ধীরে রানিং ও স্পোর্টসে ফেরার প্রস্তুতি।

চিকিৎসার প্রগ্রেশন শুধু নির্দিষ্ট সময়সূচির ওপর নির্ভর করা উচিত নয়। রোগীর উপসর্গ, হাঁটুর স্ট্যাবিলিটি, টিস্যু হিলিং এবং ফাংশনাল মাইলস্টোন বিবেচনা করে ধীরে ধীরে প্রগ্রেস করা উচিত।

কখন MCL সার্জারি বিবেচনা করা হতে পারে?

সাধারণ isolated MCL স্প্রেইনে সার্জারি প্রত্যাশিত চিকিৎসা নয়।

তবে নির্বাচিত কিছু পরিস্থিতিতে MCL রিপেয়ার বা রিকনস্ট্রাকশন বিবেচনা করা হতে পারে, যেমন:

  1. উল্লেখযোগ্য combined ligament injury।
  2. যথাযথ রিহ্যাবিলিটেশনের পরও স্থায়ী মিডিয়াল বা ভ্যালগাস ইনস্ট্যাবিলিটি।
  3. জটিল multi-ligament knee trauma।
  4. এমন কিছু নির্দিষ্ট টিয়ার প্যাটার্ন যেখানে লিগামেন্ট ডিসপ্লেসড হয়েছে বা ভালোভাবে হিল করার সম্ভাবনা কম।

MCL ইনজুরিতে ফিজিওথেরাপির ভূমিকা

MCL ইনজুরির পর হাঁটুর মুভমেন্ট, স্ট্রেংথ, স্ট্যাবিলিটি এবং ফাংশন পুনরুদ্ধারে ফিজিওথেরাপির গুরুত্বপূর্ণ ভূমিকা রয়েছে।

রিহ্যাবিলিটেশন প্রোগ্রাম ইনজুরির তীব্রতা, উপসর্গ, লিগামেন্টের স্ট্যাবিলিটি, অন্যান্য হাঁটুর ইনজুরি এবং রোগী ভবিষ্যতে কোন ধরনের অ্যাক্টিভিটিতে ফিরতে চান তার ওপর ভিত্তি করে ব্যক্তিভিত্তিক হওয়া উচিত।

  1. ব্যথা ও ফোলা কমানো:
    প্রাথমিক পর্যায়ে হিলিং লিগামেন্টকে সুরক্ষা দেওয়ার পাশাপাশি ব্যথা ও ফোলা নিয়ন্ত্রণে গুরুত্ব দেওয়া হয়।
    বারবার ভ্যালগাস স্ট্রেসের কারণে ইনজুরড MCL যেন পুনরায় ইরিটেটেড না হয়, সেজন্য সাময়িকভাবে কিছু অ্যাক্টিভিটি মডিফাই করতে হতে পারে।
  2. হাঁটুর রেঞ্জ অব মোশন পুনরুদ্ধার:
    ব্যথা ও ফোলার কারণে হাঁটুর মুভমেন্ট কমে যেতে পারে।

MCL রিহ্যাবিলিটেশনের স্টেজ

প্রতিটি MCL ইনজুরির জন্য রিহ্যাবিলিটেশন প্ল্যান আলাদা হওয়া উচিত। তবে সাধারণভাবে রিহ্যাবিলিটেশনকে কয়েকটি স্টেজে বোঝানো যায়।

স্টেজ ১: হাঁটুকে সুরক্ষা দেওয়া ও বেসিক ফাংশন ফিরিয়ে আনা

প্রাথমিক রিহ্যাবিলিটেশনে ইনজুরড লিগামেন্টকে সুরক্ষা দেওয়ার পাশাপাশি অপ্রয়োজনীয়ভাবে মুভমেন্ট ও মাসল ফাংশন হারানো প্রতিরোধ করা হয়।

লক্ষ্য হতে পারে:

মাঝারি বা গুরুতর কিছু ইনজুরিতে হাঁটুর স্ট্যাবিলিটি ও উপসর্গ অনুযায়ী সাময়িকভাবে ব্রেস বা ওয়াকিং এইড ব্যবহার করার পরামর্শ দেওয়া হতে পারে।

স্টেজ ২: স্ট্রেংথ ও মুভমেন্ট পুনর্গঠন

ব্যথা কমে আসা এবং লিগামেন্ট বেশি লোড সহ্য করতে পারার সঙ্গে সঙ্গে রিহ্যাবিলিটেশন আরও অ্যাক্টিভ হয়।

বেসিক ফাংশন উন্নত হওয়ার পর রিহ্যাবিলিটেশন আরও চ্যালেঞ্জিং টাস্কের দিকে প্রগ্রেস করা হয়।

এর মধ্যে থাকতে পারে:

  1. হাই-লোড স্ট্রেংথ ট্রেনিং।
  2. সিঙ্গেল-লেগ স্ট্রেংথেনিং।
  3. ডায়নামিক ব্যালেন্স।

স্টেজ ৪: রানিং, অ্যাজিলিটি ও স্পোর্টস-স্পেসিফিক রিহ্যাবিলিটেশন

হাই-লেভেল অ্যাক্টিভিটি বা খেলাধুলায় ফিরতে ইচ্ছুক রোগীদের ক্ষেত্রে ধীরে ধীরে যোগ করা হতে পারে:

  1. রানিং।
  2. অ্যাক্সেলারেশন ও ডেসেলারেশন।
  3. ল্যাটারাল মুভমেন্ট।
  4. চেঞ্জ-অব-ডিরেকশন ড্রিল।

কোন মুভমেন্ট কখন এবং কতটা ইনটেনসিটিতে করা হবে, তা রোগীর রিকভারি এবং খেলাধুলার ধরন অনুযায়ী নির্ধারণ করা উচিত।

স্টেজ ৫: রিটার্ন-টু-স্পোর্ট অ্যাসেসমেন্ট

আনরেস্ট্রিক্টেড স্পোর্টসে ফেরার আগে ক্লিনিশিয়ান মূল্যায়ন করতে পারেন:

  1. ব্যথা ও ফোলা।
  2. ফুল বা কাছাকাছি ফুল রেঞ্জ অব মোশন।
  3. মিডিয়াল হাঁটুর স্ট্যাবিলিটি।
  4. লোয়ার লিম্ব স্ট্রেংথ।
  5. সিঙ্গেল-লেগ কন্ট্রোল।
  6. প্রয়োজন অনুযায়ী হপ বা জাম্প পারফরম্যান্স।

রিটার্ন-টু-স্পোর্টের সিদ্ধান্ত শুধু ক্যালেন্ডারের সময় নয়, ক্লিনিক্যাল ফাইন্ডিংস এবং ফাংশনাল পারফরম্যান্স একসঙ্গে বিবেচনা করে নেওয়া উচিত।

MCL সার্জারির আগে ফিজিওথেরাপি

অধিকাংশ isolated MCL ইনজুরিতে সার্জারি লাগে না। তাই প্রি-অপারেটিভ রিহ্যাবিলিটেশন সাধারণত জটিল বা combined injury-এর ক্ষেত্রে প্রাসঙ্গিক, যেখানে সার্জারির সিদ্ধান্ত ইতোমধ্যে নেওয়া হয়েছে।

উপযুক্ত ক্ষেত্রে প্রি-অপারেটিভ ফিজিওথেরাপির লক্ষ্য হতে পারে:

  1. হাঁটুর ফোলা কমানো।
  2. হাঁটুর মুভমেন্ট পুনরুদ্ধার।
  3. কোয়াড্রিসেপস অ্যাক্টিভেশন উন্নত করা।

সঠিক পরিকল্পনা নির্ভর করবে হাঁটুর অন্য কোন স্ট্রাকচার ইনজুরড হয়েছে এবং সার্জনের নির্দেশনার ওপর।

MCL সার্জারির পর ফিজিওথেরাপি

যখন MCL রিপেয়ার বা রিকনস্ট্রাকশন প্রয়োজন হয়, তখন হাঁটুর ফাংশন ফিরিয়ে আনতে স্ট্রাকচার্ড পোস্ট-অপারেটিভ ফিজিওথেরাপি গুরুত্বপূর্ণ।

প্রাথমিক রিহ্যাবিলিটেশনে গুরুত্ব দেওয়া হতে পারে:

  1. সার্জিক্যাল রিপেয়ারকে প্রোটেক্ট করা।

পরবর্তী পর্যায়ে স্ট্রেংথেনিং, ব্যালেন্স, ফাংশনাল অ্যাক্টিভিটি, রানিং এবং স্পোর্টস-স্পেসিফিক টাস্কের দিকে প্রগ্রেস করা হয়।

কোন ধরনের সার্জারি হয়েছে এবং অন্য কোনো স্ট্রাকচার ইনজুরড ছিল কি না তার ওপর পোস্ট-অপারেটিভ রেস্ট্রিকশন উল্লেখযোগ্যভাবে ভিন্ন হতে পারে। তাই জেনেরিক MCL প্রোগ্রামের পরিবর্তে সার্জনের নির্ধারিত সতর্কতা ও নির্দেশনা অনুযায়ী রিহ্যাবিলিটেশন হওয়া উচিত।

MCL ইনজুরির পর বাসায় এক্সারসাইজ করা যায় কি?

হোম এক্সারসাইজ MCL রিহ্যাবিলিটেশনের গুরুত্বপূর্ণ অংশ হতে পারে। তবে কোন এক্সারসাইজ উপযুক্ত, তা নির্ভর করে ইনজুরির তীব্রতা, হাঁটুর স্ট্যাবিলিটি, অন্য কোনো associated injury এবং হিলিংয়ের বর্তমান স্টেজের ওপর।

উপযুক্ত রোগীর ক্ষেত্রে প্রাথমিক রিহ্যাবিলিটেশন স্টেজে ফিজিওথেরাপিস্ট নিচের মতো কিছু এক্সারসাইজ দিতে পারেন:

  1. হিল স্লাইডস:
    আরামদায়ক সীমার মধ্যে গোড়ালি শরীরের দিকে স্লাইড করে ধীরে হাঁটু ভাঁজ করুন, এরপর আবার শুরুর অবস্থানে ফিরুন।
    উপকারিতা: ধীরে ধীরে নি ফ্লেক্সন ও হাঁটুর মোবিলিটি ফিরিয়ে আনতে সাহায্য করে।
  2. কোয়াড্রিসেপস সেটস:
    পা সাপোর্ট দিয়ে সোজা রেখে সামনের উরুর মাসল ধীরে টাইট করুন। কয়েক সেকেন্ড ধরে রেখে রিল্যাক্স করুন।
    উপকারিতা: কোয়াড্রিসেপস অ্যাক্টিভেশন ফিরিয়ে আনতে সাহায্য করে।
  3. স্ট্রেইট লেগ রেইজ:
    পর্যাপ্ত কোয়াড্রিসেপস কন্ট্রোল থাকলে হাঁটু সোজা রেখে পা ধীরে ওপরে তুলুন এবং নিয়ন্ত্রণের সঙ্গে নিচে নামান।
    উপকারিতা: হাঁটুতে অতিরিক্ত মুভমেন্ট না দিয়ে লোয়ার লিম্ব স্ট্রেংথ বজায় রাখতে সাহায্য করে।
  4. অ্যাঙ্কেল পাম্পস:
    বিশ্রামের সময় গোড়ালি বারবার ওপরে ও নিচে নাড়ান।
    উপকারিতা: কম অ্যাক্টিভিটির সময় লোয়ার লিম্বের মুভমেন্ট বজায় রাখতে সাহায্য করে।

এগুলো সাধারণ উদাহরণ, সবার জন্য একই MCL রিহ্যাবিলিটেশন প্রোগ্রাম নয়।

উল্লেখযোগ্য ইনস্ট্যাবিলিটি, তীব্র ব্যথা, অন্য লিগামেন্ট ইনজুরি অথবা পোস্ট-অপারেটিভ রেস্ট্রিকশন থাকলে সম্পূর্ণ ভিন্ন এক্সারসাইজ প্ল্যান প্রয়োজন হতে পারে।

MCL ইনজুরির পর কী কী এড়িয়ে চলা উচিত?

হিলিং MCL-এর ওপর বারবার অতিরিক্ত স্ট্রেস সৃষ্টি করে এমন অ্যাক্টিভিটি হাঁটু পর্যাপ্তভাবে রিকভার না করা পর্যন্ত এড়িয়ে চলা উচিত।

ইনজুরির তীব্রতা অনুযায়ী এড়িয়ে চলার বিষয়গুলোর মধ্যে থাকতে পারে:

  1. খুব দ্রুত কন্টাক্ট স্পোর্টসে ফিরে যাওয়া।
  2. বারবার সাইড-টু-সাইড স্ট্রেস।
  3. হঠাৎ কাটিং বা পিভটিং।
  4. পর্যাপ্ত স্ট্রেংথ ও কন্ট্রোল ফেরার আগে রানিং করা।
  5. খুব দ্রুত জাম্পিং বা ল্যান্ডিং ড্রিল শুরু করা।
  6. চলমান ইনস্ট্যাবিলিটি উপেক্ষা করা।
  7. শুধু ব্যথা কমেছে বলে দ্রুত অ্যাক্টিভিটি প্রগ্রেস করা।
  8. এমন aggressive exercise করা যা হাঁটুকে বারবার ভ্যালগাস পজিশনে ঠেলে দেয়।
  9. ইনজুরির তীব্রতা বিবেচনা না করে জেনেরিক রিহ্যাবিলিটেশন প্রোগ্রাম অনুসরণ করা।

ব্যথা কমে যাওয়া মানেই লিগামেন্ট হিলিং, হাঁটুর স্ট্যাবিলিটি এবং ফাংশনাল কন্ট্রোল পুরোপুরি ফিরে এসেছে, এমন নয়।

MCL রিহ্যাবিলিটেশন কতদিন লাগে?

সব MCL ইনজুরির জন্য একটি নির্দিষ্ট রিকভারি টাইম প্রযোজ্য নয়।

রিহ্যাবিলিটেশনের সময় নির্ভর করতে পারে:

  • MCL ইনজুরির গ্রেড
  • টিয়ারের অবস্থান ও প্যাটার্ন
  • isolated নাকি combined injury
  • হাঁটুর স্ট্যাবিলিটি
  • ব্যথা ও ফোলা

  • রিহ্যাবিলিটেশন কতটা নিয়মিত হচ্ছে
  • পেশাগত কাজের চাহিদা
  • স্পোর্টস ডিমান্ড
  • সার্জারি হয়েছে কি না

হালকা MCL স্প্রেইন সাধারণত complete tear বা combined ligament injury-এর তুলনায় অনেক দ্রুত রিকভার করে।

তবে গুরুত্বপূর্ণ নীতি হলো, প্রগ্রেশন শুধু সময়ের ওপর ভিত্তি করে করা উচিত নয়। রোগীকে আরও চ্যালেঞ্জিং অ্যাক্টিভিটিতে নেওয়ার আগে উপযুক্ত হিলিং, স্ট্যাবিলিটি, স্ট্রেংথ এবং ফাংশনাল সক্ষমতা থাকা প্রয়োজন।

MCL ইনজুরি কি প্রতিরোধ করা সম্ভব?

সব MCL ইনজুরি প্রতিরোধ করা সম্ভব নয়, বিশেষ করে অন্য খেলোয়াড়ের অনির্দেশ্য সংঘর্ষ থেকে হওয়া ইনজুরি।

তবে উপযুক্ত ট্রেনিং পরিবর্তনযোগ্য কিছু মুভমেন্ট-রিলেটেড রিস্ক ফ্যাক্টরের ওপর কন্ট্রোল উন্নত করতে সাহায্য করতে পারে।

MCL ইনজুরিতে কখন ফিজিওথেরাপিস্ট দেখাবেন?

নিচের পরিস্থিতিতে প্রফেশনাল অ্যাসেসমেন্ট নেওয়া উচিত:
