Common Locations of Hip Pain and What They May Mean

  1. Front of the Hip or Groin Pain:
    Pain deep in the front of the hip or groin may be associated with the hip joint itself.

    Possible causes include:

    1. Hip osteoarthritis
    2. Femoroacetabular impingement
    3. Labral pathology
    4. Iliopsoas-related pain
    5. Adductor or groin-related problems

    Pain may become more noticeable during walking, squatting, getting into or out of a car, prolonged sitting or activities involving deeper hip flexion.

  2. Outer or Side of the Hip Pain:
    Pain around the lateral hip, particularly over or near the greater trochanter, is commonly associated with Greater Trochanteric Pain Syndrome (GTPS).

    Gluteal tendons are often involved, and symptoms may become worse when:

    1. Walking for longer periods
    2. Climbing stairs
    3. Standing on one leg
    4. Lying on the painful side
    5. Running or increasing activity load

    Lateral hip pain should not automatically be labelled simply as "bursitis," because tendon-related structures may also contribute.

  3. Buttock or Back of the Hip Pain:
    Pain around the buttock or posterior hip can arise from several structures.

    Possible causes include:

    1. Deep gluteal muscle problems
    2. Piriformis-related symptoms
    3. Proximal hamstring problems
    4. Sacroiliac-region pain
    5. Referred pain from the lumbar spine
  4. Hip Pain With Stiffness:
    Deep hip or groin pain accompanied by progressively reduced movement may occur with Hip Osteoarthritis or other joint-related conditions.

    Patients may notice increasing difficulty putting on socks or shoes, crossing the legs, getting in and out of a car, walking longer distances or rotating the hip.

  5. Hip Pain During Running or Sports:
    Pain during running, kicking, sprinting or rapid direction changes may be associated with:
    1. Adductor strain
    2. Hip flexor strain
    3. Gluteal tendon problems
    4. Proximal hamstring injury
    5. Femoroacetabular impingement
    6. Labral injury

    The mechanism of injury and exact location of pain are important for differentiating these problems.

  6. Hip Pain With Clicking or Catching:
    Clicking around the hip is not always abnormal. However, painful clicking, catching or deep groin pain associated with movement may require assessment for intra-articular or tendon-related causes.

    Labral pathology, femoroacetabular impingement and snapping hip presentations are some possible considerations.

  7. Hip Pain Extending Down the Leg:
    Pain felt around the hip or buttock that extends down the thigh or leg, particularly when accompanied by tingling or numbness, may not originate solely from the hip joint.

    The lumbar spine, sciatic nerve or other neural structures may need to be assessed.

Common Causes of Hip Pain

Hip-region pain can originate from the joint, muscles, tendons, bursae, labrum or surrounding neural structures. The following are some of the important causes.

  1. Hip Osteoarthritis:
    Hip Osteoarthritis is a joint condition involving structural and functional changes within the hip.

    Symptoms commonly develop gradually and may include deep groin or hip pain, stiffness, reduced hip rotation and difficulty walking, climbing stairs, getting out of a chair or performing other weight-bearing activities.

    Reduced hip movement and muscle weakness can gradually affect gait and daily function.

  2. Greater Trochanteric Pain Syndrome:
    Greater Trochanteric Pain Syndrome (GTPS) is an important cause of pain around the outer side of the hip.

    It commonly involves the gluteal tendon region and may occur with irritation of nearby bursae.

    Typical symptoms include lateral hip tenderness and pain during prolonged walking, stair climbing, standing on one leg or lying on the affected side.

  3. Gluteal Tendinopathy:
    The gluteus medius and gluteus minimus tendons contribute significantly to pelvic and hip control during walking and single-leg activities.

    Tendon overload can produce pain around the outer hip, particularly with activities that repeatedly load the lateral hip.

    Rehabilitation may require progressive strengthening and appropriate load management rather than prolonged rest alone.

    Gluteal tendinopathy and GTPS overlap clinically, so they should not necessarily be treated as completely separate diagnoses in every patient.

  4. Femoroacetabular Impingement:
    Femoroacetabular Impingement (FAI) refers to a pattern in which hip morphology and movement can contribute to contact between the femur and acetabular region during certain positions.

    Patients may experience front-of-hip or groin pain, particularly during deep hip flexion, squatting, prolonged sitting or sports involving repeated hip rotation.

    FAI-related imaging findings do not automatically explain pain, so clinical symptoms and functional assessment are important.

  5. Hip Labral Injury:
    The acetabular labrum is a ring of fibrocartilage around the hip socket that contributes to joint function and stability.

    Labral pathology may occur after trauma, repetitive sports activity or alongside other hip conditions.

    Symptoms may include deep groin pain, painful clicking, catching or discomfort during twisting and hip flexion.

    Not every labral abnormality seen on imaging causes symptoms, so clinical correlation is important.

  6. Hip Flexor and Iliopsoas-Related Pain:
    The iliopsoas is one of the major hip flexor muscle-tendon complexes.

    Pain may be felt around the front of the hip or groin and may increase during repeated hip flexion, running, stair climbing or activities that require lifting the thigh.

    Muscle strain and tendon-related conditions require different rehabilitation depending on the mechanism and stage of injury.

  7. Adductor-Related Groin Pain:
    The hip adductor muscles are located along the inner thigh and contribute to hip control during walking, running and sporting movements.

    Overload or injury can cause pain around the inner thigh and groin, particularly during sprinting, kicking, changing direction or squeezing the legs together.

  8. Piriformis Syndrome / Deep Gluteal Pain:
    Piriformis-related pain may produce deep buttock discomfort and, in some patients, symptoms extending toward the posterior thigh.

    Prolonged sitting, certain hip movements and walking may aggravate symptoms.

    Because similar symptoms can arise from lumbar radiculopathy and other deep-gluteal conditions, careful differential assessment is important.

  9. Proximal Hamstring Tendinopathy or Injury:
    The proximal hamstring tendons attach near the sitting bone at the lower buttock.

    Repetitive running, sprinting, bending or prolonged sitting may aggravate tendon-related pain. Sudden sporting injuries can also cause hamstring strain or tearing.

    Patients often report pain around the lower buttock rather than directly over the side of the hip.

  10. Hip Bursitis:
    Several bursae around the hip reduce friction between nearby tissues.

    Bursal irritation can contribute to local pain and tenderness, particularly around the lateral hip.

    However, lateral hip pain is frequently more complex than isolated bursitis, so assessment should consider the gluteal tendons and surrounding structures as well.

  11. Hip Muscle Strain:
    Hip flexor, adductor, gluteal and other muscles may be injured during sprinting, kicking, sudden direction changes, slipping or unexpected loading.

    Symptoms may include localized pain, tenderness, weakness and pain during resisted movement or stretching.

    The severity of muscle injury determines the appropriate progression back to activity.

  12. Referred Pain From the Lumbar Spine:
    Not every pain felt around the hip comes from the hip joint.

    Lumbar spine problems may produce pain around the buttock, lateral hip, thigh or leg.

    Symptoms such as back pain, tingling, numbness, radiating leg pain or pain influenced strongly by spinal movement increase the need to assess the lumbar region.

  13. Hip Fracture or Significant Traumatic Injury:
    Hip and proximal femoral fractures may occur after significant trauma and are particularly important in older adults after a fall.

    Severe pain, inability to bear weight, visible leg deformity or sudden marked loss of hip function after trauma requires urgent medical assessment rather than routine physiotherapy management.

Risk Factors for Developing Hip Pain

Several factors may increase loading around the hip or contribute to particular hip disorders:

  1. Sudden increase in walking, running or sports training
  2. Repetitive running, sprinting or kicking
  3. Repeated deep squatting or hip flexion
  4. Previous hip, pelvic or lower-limb injury
  5. Reduced hip or gluteal muscle capacity
  6. Poor load management during exercise
  7. Prolonged sitting in some hip and buttock conditions
  8. Age-related joint changes
  9. Repeated occupational loading
  10. Reduced balance or lower-limb control
  11. Previous surgery or prolonged immobilization

Having one or more risk factors does not confirm the cause of hip pain. Symptoms should be interpreted together with the patient's history, examination and functional demands.

Symptoms of Hip Pain

Symptoms vary according to the structure involved. Common complaints include:

  1. Pain in the groin or front of the hip
  2. Pain on the outer side of the hip
  3. Buttock or posterior hip pain
  4. Pain while walking or climbing stairs
  5. Difficulty getting up from a chair
  6. Pain when lying on one side
  7. Hip stiffness or reduced range of motion
  8. Weakness during walking or single-leg activities
  9. Clicking, catching or painful rotation
  10. Difficulty running, squatting or participating in sport
  11. Pain extending into the thigh or leg
  12. Tingling or numbness when neural structures are involved

Hip Pain Symptoms That Should Not Be Ignored

Some hip symptoms require medical assessment rather than routine self-management alone.

Seek appropriate professional care if you experience:

  1. Severe hip or groin pain after a fall or significant trauma
  2. Inability to stand or bear weight after injury
  3. Visible deformity or abnormal leg position
  4. Rapidly increasing swelling or extensive bruising
  5. A hot, red or significantly swollen joint with fever
  6. Progressive weakness or numbness in the leg
  7. Severe persistent pain without a clear mechanical pattern
  8. Significant unexplained loss of function
  9. Persistent pain after trauma despite initial rest
  10. Sudden severe pain in a person at increased risk of fracture

These symptoms do not always indicate a serious disorder, but fracture, infection, neurological dysfunction or other pathology may need to be excluded.

Severe pain after a fall, inability to bear weight, or a hot swollen hip with fever warrants urgent medical assessment.

How Hip Pain Is Diagnosed

Accurate assessment is important because joint, tendon, muscle, lumbar and nerve-related problems can produce overlapping symptoms around the hip.

Clinical assessment may include:

  1. History of how and when symptoms began
  2. Exact location and behaviour of pain
  3. Mechanism of injury when trauma occurred
  4. Walking and gait assessment
  5. Hip range of motion
  6. Hip and lower-limb muscle strength
  7. Single-leg control and balance
  8. Tendon loading assessment when indicated
  9. Functional movements such as squat, step or sit-to-stand
  10. Hip joint provocation tests when clinically relevant
  11. Neurological screening when symptoms radiate
  12. Lumbar spine assessment when referred pain is suspected

Different presentations require different clinical tests. A patient with lateral hip tendon pain, for example, should not automatically be assessed or treated in exactly the same way as someone with deep groin pain or radiating neurological symptoms.

Imaging is not necessary for every person with hip pain.

When clinically indicated, investigations may include:

  1. X-ray
  2. MRI
  3. Musculoskeletal ultrasound
  4. Other medical investigations according to the suspected pathology

The need for imaging should be determined by the clinical presentation rather than used routinely for all hip pain.

Physiotherapy Treatment for Hip Pain

There is no single physiotherapy treatment or exercise program suitable for every type of hip pain.

Rehabilitation depends on the involved structure, duration and irritability of symptoms, range of motion, muscle capacity, walking pattern and individual functional goals.

  1. Education and Load Modification:
    Activities that repeatedly aggravate the hip should first be identified.

    Depending on the condition, temporary modification of long-distance walking, running, jumping, deep squatting, repeated stair use, prolonged sitting or high-load sports may help control symptoms.

    The goal is generally to reduce excessive load while maintaining appropriate activity rather than unnecessarily stopping all movement.

  2. Pain Management:
    An irritated hip joint, tendon or muscle may initially require strategies to reduce aggravating load and allow movement within an appropriate level of symptoms.

  3. Mobility and Range-of-Motion Training:
    Hip stiffness may affect walking, sitting, dressing and other functional tasks.

কোন গঠন জড়িত তার ওপর ভিত্তি করে হিপ সমস্যায় মৃদু বা তীক্ষ্ণ ব্যথা, স্টিফনেস, দুর্বলতা, রেঞ্জ অব মোশন কমে যাওয়া, টেন্ডারনেস, ক্লিকিং, ক্যাচিং, হাঁটতে অসুবিধা বা ওয়েট-বেয়ারিংয়ের সময় ব্যথা হতে পারে।

ব্যথার অবস্থান সম্ভাব্য কারণ সম্পর্কে গুরুত্বপূর্ণ ধারণা দিতে পারে, তবে শুধু ব্যথার স্থান দেখে নিশ্চিত ডায়াগনসিস করা যায় না।

হিপ পেইনের সাধারণ স্থান এবং এগুলো কী নির্দেশ করতে পারে

  1. হিপের সামনের অংশ বা গ্রোইনে ব্যথা:
    হিপের সামনে বা গ্রোইনের গভীরে ব্যথা হলে হিপ জয়েন্ট নিজেই জড়িত থাকতে পারে।

    সম্ভাব্য কারণের মধ্যে রয়েছে:

    1. হিপ অস্টিওআর্থ্রাইটিস
    2. ফেমোরোঅ্যাসিটাবুলার ইম্পিঞ্জমেন্ট
    3. ল্যাব্রাল প্যাথোলজি

    হাঁটা, স্কোয়াট করা, গাড়িতে ওঠানামা, দীর্ঘ সময় বসে থাকা বা হিপকে বেশি ফ্লেক্স করতে হয় এমন অ্যাক্টিভিটির সময় ব্যথা বেশি অনুভূত হতে পারে।

  2. হিপের বাইরের বা পাশের দিকে ব্যথা:
    ল্যাটেরাল হিপে, বিশেষ করে গ্রেটার ট্রোক্যান্টারের আশপাশে ব্যথা হলে Greater Trochanteric Pain Syndrome (GTPS) বিবেচনা করা যেতে পারে।

    এতে গ্লুটিয়াল টেনডন প্রায়ই জড়িত থাকে এবং কিছু ক্ষেত্রে আশপাশের বার্সাও ইরিটেটেড হতে পারে।

    উপসর্গ বাড়তে পারে:

    1. দীর্ঘ সময় হাঁটার সময়
    2. সিঁড়ি ওঠার সময়
    3. এক পায়ে দাঁড়ালে
    4. ব্যথার পাশের ওপর শুলে

    ল্যাটেরাল হিপ পেইনকে শুধু “বার্সাইটিস” হিসেবে ধরে নেওয়া উচিত নয়, কারণ টেনডন-সম্পর্কিত গঠনও গুরুত্বপূর্ণ ভূমিকা রাখতে পারে।

  3. বাটক বা হিপের পেছনের দিকে ব্যথা:
    বাটক বা পোস্টেরিয়র হিপ পেইন একাধিক গঠন থেকে আসতে পারে।

    সম্ভাব্য কারণ:

    1. ডিপ গ্লুটিয়াল মাসল সমস্যা
    2. পিরিফরমিস-সম্পর্কিত উপসর্গ
    3. প্রোক্সিমাল হ্যামস্ট্রিং সমস্যা
    4. স্যাক্রোইলিয়াক রিজিয়নের ব্যথা
    5. লাম্বার স্পাইন থেকে রেফার্ড পেইন
  4. স্টিফনেসসহ হিপ পেইন:
    হিপ বা গ্রোইনের গভীরে ব্যথার সঙ্গে যদি ধীরে ধীরে মুভমেন্ট কমতে থাকে, তাহলে Hip Osteoarthritis বা অন্যান্য জয়েন্ট-সম্পর্কিত সমস্যা থাকতে পারে।

    রোগীরা মোজা বা জুতা পরা, পা ক্রস করা, গাড়িতে ওঠানামা, দীর্ঘ দূরত্ব হাঁটা অথবা হিপ ঘোরাতে ক্রমশ বেশি অসুবিধা অনুভব করতে পারেন।

  5. দৌড়ানো বা খেলাধুলার সময় হিপ পেইন:
    রানিং, কিকিং, স্প্রিন্টিং বা দ্রুত দিক পরিবর্তনের সময় ব্যথা হলে নিচের সমস্যাগুলো বিবেচনা করা যেতে পারে:
    1. অ্যাডাক্টর স্ট্রেইন
    2. হিপ ফ্লেক্সর স্ট্রেইন
    3. গ্লুটিয়াল টেনডন সমস্যা
    4. প্রোক্সিমাল হ্যামস্ট্রিং ইনজুরি
    5. ফেমোরোঅ্যাসিটাবুলার ইম্পিঞ্জমেন্ট
    6. ল্যাব্রাল ইনজুরি
    7. অন্যান্য স্পোর্টস-সম্পর্কিত হিপ বা গ্রোইন সমস্যা

    কোন মুভমেন্টে ইনজুরি হয়েছে এবং ব্যথার সঠিক অবস্থান কোথায়, তা কন্ডিশনগুলো আলাদা করতে গুরুত্বপূর্ণ।

  6. ক্লিকিং বা ক্যাচিংসহ হিপ পেইন:
    হিপে ক্লিকিং সবসময় অস্বাভাবিক নয়। তবে ক্লিকিং যদি ব্যথাযুক্ত হয়, ক্যাচিং অনুভূত হয় অথবা হিপ মুভমেন্টের সঙ্গে গভীর গ্রোইন পেইন থাকে, তাহলে ইনট্রা-আর্টিকুলার বা টেনডন-সম্পর্কিত কারণ মূল্যায়ন করা প্রয়োজন হতে পারে।

    ল্যাব্রাল প্যাথোলজি, ফেমোরোঅ্যাসিটাবুলার ইম্পিঞ্জমেন্ট এবং snapping hip presentation সম্ভাব্য কিছু কারণ।

  7. হিপ পেইন পায়ের দিকে ছড়িয়ে যাওয়া:
    হিপ বা বাটক থেকে ব্যথা উরু বা পায়ের নিচের দিকে ছড়িয়ে গেলে, বিশেষ করে ঝিনঝিন বা অসাড়তা থাকলে, সমস্যাটি শুধু হিপ জয়েন্ট থেকে নাও আসতে পারে।

    এ ধরনের ক্ষেত্রে লাম্বার স্পাইন, সায়াটিক নার্ভ অথবা অন্যান্য নিউরাল স্ট্রাকচারও অ্যাসেসমেন্ট করা প্রয়োজন হতে পারে।

হিপ পেইনের সাধারণ কারণসমূহ

হিপ অঞ্চলের ব্যথা জয়েন্ট, মাসল, টেনডন, বার্সা, ল্যাব্রাম বা আশপাশের নিউরাল স্ট্রাকচার থেকে তৈরি হতে পারে। নিচে কয়েকটি গুরুত্বপূর্ণ কারণ আলোচনা করা হলো।

  1. হিপ অস্টিওআর্থ্রাইটিস:
    Hip Osteoarthritis হলো হিপ জয়েন্টের একটি কন্ডিশন, যেখানে জয়েন্টে স্ট্রাকচারাল ও ফাংশনাল পরিবর্তন তৈরি হয়।

    উপসর্গ সাধারণত ধীরে ধীরে শুরু হয় এবং এর মধ্যে গ্রোইন বা হিপের গভীরে ব্যথা, স্টিফনেস, হিপ রোটেশন কমে যাওয়া, হাঁটতে অসুবিধা, সিঁড়ি ওঠা বা চেয়ার থেকে উঠতে সমস্যা থাকতে পারে।

    হিপ মুভমেন্ট কমে যাওয়া এবং মাসল দুর্বলতা ধীরে ধীরে গেইট ও দৈনন্দিন ফাংশনকে প্রভাবিত করতে পারে।

  2. Greater Trochanteric Pain Syndrome:
    Greater Trochanteric Pain Syndrome বা GTPS হিপের বাইরের দিকের ব্যথার একটি গুরুত্বপূর্ণ কারণ।

    এতে সাধারণত গ্লুটিয়াল টেনডন অঞ্চল জড়িত থাকে এবং কখনও আশপাশের বার্সা ইরিটেটেড হতে পারে।

    দীর্ঘ সময় হাঁটা, সিঁড়ি ওঠা, এক পায়ে দাঁড়ানো অথবা আক্রান্ত পাশের ওপর শুলে ল্যাটেরাল হিপে টেন্ডারনেস ও ব্যথা বাড়তে পারে।

  3. গ্লুটিয়াল টেন্ডিনোপ্যাথি:
    গ্লুটিয়াস মিডিয়াস ও গ্লুটিয়াস মিনিমাস টেনডন হাঁটা এবং এক পায়ে ভর দিয়ে দাঁড়ানোর সময় পেলভিস ও হিপ কন্ট্রোলে গুরুত্বপূর্ণ ভূমিকা পালন করে।

    অতিরিক্ত বা বারবার লোড পড়লে ল্যাটেরাল হিপে টেনডন-সম্পর্কিত ব্যথা তৈরি হতে পারে।

    রিহ্যাবিলিটেশনে দীর্ঘ সময় সম্পূর্ণ বিশ্রামের পরিবর্তে উপযুক্ত লোড ম্যানেজমেন্ট এবং প্রগ্রেসিভ স্ট্রেন্থেনিং প্রয়োজন হতে পারে।

    গ্লুটিয়াল টেন্ডিনোপ্যাথি ও GTPS-এর মধ্যে উল্লেখযোগ্য ক্লিনিক্যাল ওভারল্যাপ রয়েছে, তাই সব ক্ষেত্রে এগুলোকে সম্পূর্ণ আলাদা সমস্যা হিসেবে দেখা হয় না।

  4. ফেমোরোঅ্যাসিটাবুলার ইম্পিঞ্জমেন্ট:
    Femoroacetabular Impingement বা FAI এমন একটি প্যাটার্ন, যেখানে হিপের গঠন এবং নির্দিষ্ট মুভমেন্টের সময় ফিমার ও অ্যাসিটাবুলার অঞ্চলের মধ্যে কন্টাক্ট উপসর্গের সঙ্গে সম্পর্কিত হতে পারে।

    হিপের সামনে বা গ্রোইনে ব্যথা হতে পারে, বিশেষ করে ডিপ হিপ ফ্লেক্সন, স্কোয়াটিং, দীর্ঘ সময় বসে থাকা অথবা বারবার হিপ রোটেশন করতে হয় এমন খেলাধুলায়।

    ইমেজিংয়ে FAI-সম্পর্কিত পরিবর্তন দেখা গেলেই যে সেটি ব্যথার কারণ, এমন নয়। ক্লিনিক্যাল উপসর্গ ও ফাংশনাল অ্যাসেসমেন্টের সঙ্গে মিলিয়ে দেখা গুরুত্বপূর্ণ।

  5. হিপ ল্যাব্রাল ইনজুরি:
    অ্যাসিটাবুলার ল্যাব্রাম হলো হিপ সকেটের চারপাশে থাকা ফাইব্রোকার্টিলেজের একটি রিং, যা জয়েন্ট ফাংশন ও স্ট্যাবিলিটিতে ভূমিকা রাখে।

    ট্রমা, রিপিটেড স্পোর্টস অ্যাক্টিভিটি অথবা অন্যান্য হিপ কন্ডিশনের সঙ্গে ল্যাব্রাল প্যাথোলজি থাকতে পারে।

    উপসর্গের মধ্যে গ্রোইনের গভীরে ব্যথা, ব্যথাযুক্ত ক্লিকিং, ক্যাচিং অথবা টুইস্টিং ও হিপ ফ্লেক্সনের সময় অস্বস্তি থাকতে পারে।

    ইমেজিংয়ে ল্যাব্রাল পরিবর্তন দেখা গেলেই যে তা উপসর্গের কারণ, এমন নয়। ক্লিনিক্যাল ফাইন্ডিংসের সঙ্গে এর সম্পর্ক মূল্যায়ন করা গুরুত্বপূর্ণ।

  6. হিপ ফ্লেক্সর ও ইলিওপসোয়াস-সম্পর্কিত ব্যথা:
    ইলিওপসোয়াস হিপের প্রধান ফ্লেক্সর মাসল-টেনডন কমপ্লেক্সগুলোর একটি।

    হিপের সামনে বা গ্রোইন অঞ্চলে ব্যথা হতে পারে এবং বারবার হিপ ফ্লেক্সন, দৌড়ানো, সিঁড়ি ওঠা বা উরু উপরের দিকে তুলতে হয় এমন অ্যাক্টিভিটির সময় উপসর্গ বাড়তে পারে।

    মাসল স্ট্রেইন এবং টেনডন-সম্পর্কিত সমস্যার রিহ্যাবিলিটেশন ইনজুরির মেকানিজম ও স্টেজ অনুযায়ী ভিন্ন হতে পারে।

  7. অ্যাডাক্টর-রিলেটেড গ্রোইন পেইন:
    হিপ অ্যাডাক্টর মাসলগুলো উরুর ভেতরের দিকে থাকে এবং হাঁটা, দৌড়ানো ও বিভিন্ন স্পোর্টস মুভমেন্টের সময় হিপ কন্ট্রোলে ভূমিকা রাখে।

    অতিরিক্ত লোড বা ইনজুরির কারণে ইনার থাই এবং গ্রোইনে ব্যথা হতে পারে, বিশেষ করে স্প্রিন্টিং, কিকিং, দ্রুত দিক পরিবর্তন অথবা দুই পা একসঙ্গে চাপ দেওয়ার সময়।

  8. Piriformis Syndrome / Deep Gluteal Pain:
    পিরিফরমিস-সম্পর্কিত সমস্যায় বাটকের গভীরে ব্যথা হতে পারে এবং কিছু রোগীর ক্ষেত্রে ব্যথা পেছনের উরুর দিকে ছড়িয়ে যেতে পারে।

    দীর্ঘ সময় বসে থাকা, নির্দিষ্ট হিপ মুভমেন্ট বা হাঁটার সময় উপসর্গ বাড়তে পারে।

    লাম্বার রেডিকুলোপ্যাথি এবং অন্যান্য ডিপ গ্লুটিয়াল কন্ডিশনও কাছাকাছি ধরনের উপসর্গ তৈরি করতে পারে, তাই ডিফারেনশিয়াল অ্যাসেসমেন্ট গুরুত্বপূর্ণ।

  9. প্রোক্সিমাল হ্যামস্ট্রিং টেন্ডিনোপ্যাথি বা ইনজুরি:
    প্রোক্সিমাল হ্যামস্ট্রিং টেনডন বাটকের নিচের দিকে sitting bone-এর কাছে সংযুক্ত থাকে।

    বারবার দৌড়ানো, স্প্রিন্টিং, সামনে ঝোঁকা বা দীর্ঘ সময় বসে থাকার সময় টেনডন-সম্পর্কিত ব্যথা বাড়তে পারে। আবার আকস্মিক স্পোর্টস ইনজুরিতেও হ্যামস্ট্রিং স্ট্রেইন বা টিয়ার হতে পারে।

    রোগীরা সাধারণত হিপের বাইরের দিকে নয়, বরং বাটকের নিচের দিকে ব্যথা অনুভব করেন।

  10. হিপ বার্সাইটিস:
    হিপের আশপাশে কয়েকটি বার্সা থাকে, যা নিকটবর্তী টিস্যুর মধ্যে ঘর্ষণ কমাতে সাহায্য করে।

    বার্সা ইরিটেটেড হলে বিশেষ করে ল্যাটেরাল হিপে লোকালাইজড ব্যথা ও টেন্ডারনেস হতে পারে।

    তবে ল্যাটেরাল হিপ পেইন অনেক সময় শুধু isolated bursitis নয়; গ্লুটিয়াল টেনডন ও আশপাশের গঠনও মূল্যায়ন করা উচিত।

  11. হিপ মাসল স্ট্রেইন:
    হিপ ফ্লেক্সর, অ্যাডাক্টর, গ্লুটিয়াল এবং অন্যান্য মাসল স্প্রিন্টিং, কিকিং, হঠাৎ দিক পরিবর্তন, পিছলে যাওয়া বা অপ্রত্যাশিত লোডের সময় ইনজুরড হতে পারে।

    লোকালাইজড ব্যথা, টেন্ডারনেস, দুর্বলতা এবং রেজিস্টেড মুভমেন্ট বা স্ট্রেচিংয়ের সময় ব্যথা হতে পারে।

    মাসল ইনজুরির তীব্রতার ওপর ভিত্তি করে রিটার্ন-টু-অ্যাক্টিভিটি প্রগ্রেশন নির্ধারণ করা হয়।

  12. লাম্বার স্পাইন থেকে রেফার্ড পেইন:
    হিপের আশপাশে অনুভূত সব ব্যথাই হিপ জয়েন্ট থেকে আসে না।

    লাম্বার স্পাইনের সমস্যা বাটক, ল্যাটেরাল হিপ, উরু বা পায়ে ব্যথা তৈরি করতে পারে।

    ব্যাক পেইন, ঝিনঝিন, অসাড়তা, পায়ের দিকে ছড়ানো ব্যথা অথবা স্পাইনাল মুভমেন্টের সঙ্গে উপসর্গ উল্লেখযোগ্যভাবে পরিবর্তিত হলে লাম্বার রিজিয়নও অ্যাসেসমেন্ট করা প্রয়োজন।

  13. হিপ ফ্র্যাকচার বা উল্লেখযোগ্য ট্রমাটিক ইনজুরি:
    গুরুতর ট্রমার পর হিপ বা প্রোক্সিমাল ফিমোরাল ফ্র্যাকচার হতে পারে। বিশেষ করে বয়স্ক ব্যক্তিদের পড়ে যাওয়ার পর এটি গুরুত্বপূর্ণ একটি সম্ভাবনা।

    ইনজুরির পর তীব্র ব্যথা, পায়ে ভর দিতে না পারা, পায়ের অবস্থানে দৃশ্যমান পরিবর্তন অথবা হিপের ফাংশন হঠাৎ উল্লেখযোগ্যভাবে কমে গেলে রুটিন ফিজিওথেরাপির পরিবর্তে জরুরি মেডিক্যাল অ্যাসেসমেন্ট প্রয়োজন।

হিপ পেইনের ঝুঁকির কারণসমূহ

কিছু বিষয় হিপের ওপর লোড বাড়াতে পারে অথবা নির্দিষ্ট হিপ কন্ডিশনে ভূমিকা রাখতে পারে:

  1. হঠাৎ হাঁটা, দৌড়ানো বা স্পোর্টস ট্রেনিংয়ের পরিমাণ বাড়ানো
  2. বারবার রানিং, স্প্রিন্টিং বা কিকিং
  3. বারবার ডিপ স্কোয়াট বা বেশি হিপ ফ্লেক্সন
  4. আগের হিপ, পেলভিস বা লোয়ার-লিম্ব ইনজুরি
  5. হিপ বা গ্লুটিয়াল মাসল ক্যাপাসিটি কম থাকা
  6. এক্সারসাইজে ভুল লোড ম্যানেজমেন্ট
  7. কিছু হিপ ও বাটক কন্ডিশনে দীর্ঘ সময় বসে থাকা
  8. বয়সজনিত জয়েন্ট পরিবর্তন
  9. পুনরাবৃত্তিমূলক পেশাগত লোড
  10. ব্যালান্স বা লোয়ার-লিম্ব কন্ট্রোল কম থাকা
  11. পূর্বের সার্জারি বা দীর্ঘ সময় ইমোবিলাইজেশন

এক বা একাধিক রিস্ক ফ্যাক্টর থাকলেই হিপ পেইনের নির্দিষ্ট কারণ নিশ্চিত হয় না। রোগীর ইতিহাস, ক্লিনিক্যাল এক্সামিনেশন এবং ফাংশনাল ডিমান্ডের সঙ্গে মিলিয়ে উপসর্গ মূল্যায়ন করা প্রয়োজন।

হিপ পেইনের সাধারণ উপসর্গ

মূল গঠন বা কন্ডিশনের ওপর ভিত্তি করে উপসর্গ ভিন্ন হতে পারে। সাধারণ উপসর্গগুলোর মধ্যে রয়েছে:

  1. গ্রোইন বা হিপের সামনে ব্যথা
  2. হিপের বাইরের দিকে ব্যথা
  3. বাটক বা হিপের পেছনের দিকে ব্যথা
  4. হাঁটা বা সিঁড়ি ওঠার সময় ব্যথা
  5. চেয়ার থেকে উঠতে অসুবিধা
  6. এক পাশে শুলে ব্যথা
  7. হিপ স্টিফনেস বা রেঞ্জ অব মোশন কমে যাওয়া
  8. হাঁটা বা এক পায়ে ভর দেওয়ার সময় দুর্বলতা
  9. ক্লিকিং, ক্যাচিং বা ব্যথাযুক্ত রোটেশন
  10. দৌড়ানো, স্কোয়াট করা বা খেলাধুলায় অসুবিধা
  11. ব্যথা উরু বা পায়ের দিকে ছড়িয়ে যাওয়া
  12. নিউরাল স্ট্রাকচার জড়িত থাকলে ঝিনঝিন বা অসাড়তা

হিপ পেইনের যেসব উপসর্গ অবহেলা করা উচিত নয়

কিছু হিপ উপসর্গ সাধারণ সেলফ-ম্যানেজমেন্টের পরিবর্তে মেডিক্যাল অ্যাসেসমেন্টের প্রয়োজন হতে পারে।

নিচের উপসর্গগুলোর কোনোটি থাকলে যথাযথ প্রফেশনাল কেয়ার নিন:

  1. পড়ে যাওয়া বা উল্লেখযোগ্য ট্রমার পর তীব্র হিপ বা গ্রোইন পেইন
  2. ইনজুরির পর দাঁড়াতে বা পায়ে ভর দিতে না পারা
  3. দৃশ্যমান ডিফর্মিটি বা পায়ের অস্বাভাবিক অবস্থান
  4. দ্রুত বাড়তে থাকা ফোলা বা ব্যাপক ব্রুইজিং
  5. জ্বরের সঙ্গে হিপ জয়েন্ট গরম, লাল বা উল্লেখযোগ্যভাবে ফুলে যাওয়া
  6. পায়ের দুর্বলতা বা অসাড়তা ক্রমশ বাড়তে থাকা
  7. স্পষ্ট মেকানিক্যাল প্যাটার্ন ছাড়াই দীর্ঘস্থায়ী তীব্র ব্যথা
  8. ব্যাখ্যাতীতভাবে ফাংশন উল্লেখযোগ্যভাবে কমে যাওয়া
  9. প্রাথমিক বিশ্রামের পরও ট্রমা-পরবর্তী ব্যথা থেকে যাওয়া
  10. ফ্র্যাকচারের ঝুঁকি বেশি এমন ব্যক্তির হঠাৎ তীব্র ব্যথা

এসব উপসর্গ সবসময় গুরুতর কোনো সমস্যা নির্দেশ করে না, তবে ফ্র্যাকচার, ইনফেকশন, নিউরোলজিক্যাল ডিসফাংশন বা অন্য কোনো প্যাথোলজি বাদ দেওয়া প্রয়োজন হতে পারে।

পড়ে যাওয়ার পর তীব্র ব্যথা, পায়ে ভর দিতে না পারা অথবা জ্বরের সঙ্গে হিপ গরম ও ফুলে গেলে জরুরি মেডিক্যাল অ্যাসেসমেন্ট নিন।

হিপ পেইন কীভাবে নির্ণয় করা হয়

জয়েন্ট, টেনডন, মাসল, লাম্বার স্পাইন এবং নার্ভ-সম্পর্কিত সমস্যা অনেক সময় হিপ অঞ্চলে কাছাকাছি ধরনের উপসর্গ তৈরি করতে পারে। তাই সঠিক ক্লিনিক্যাল অ্যাসেসমেন্ট গুরুত্বপূর্ণ।

অ্যাসেসমেন্টের মধ্যে থাকতে পারে:

  1. কখন ও কীভাবে উপসর্গ শুরু হয়েছে তার ইতিহাস
  2. ব্যথার নির্দিষ্ট অবস্থান ও আচরণ
  3. ট্রমা থাকলে ইনজুরির মেকানিজম
  4. হাঁটা ও গেইট অ্যাসেসমেন্ট
  5. হিপ রেঞ্জ অব মোশন
  6. হিপ ও লোয়ার-লিম্ব মাসল স্ট্রেন্থ
